目的:了解本院临床分离铜绿假单胞菌产生金属β-内酰胺酶的耐药现况与其耐药基因携带率,并分析与亚胺培南耐药关系,为预防临床感染及治疗提供重要指导.方法:临床收集2010-2012年患者标本并分离培养铜绿假单胞菌,菌株耐药性采用纸片扩散法进行检测,而金属β-内酰胺酶采用双纸片协同法检测,运用聚合酶链反应的方法检测其耐药基因.结果:在216株铜绿假单胞菌中检测出80株产金属β-内酰胺酶,占37.03%,其IMP与VIM基因检出率分别为28.75%(23/80)与11.25%(9/80),其中9株携带VIM基因的菌株都对亚胺培南抗生素耐药,而Ⅰ类整合子检出率高达82.5%(66/80),其中亚胺培南耐药菌株中Ⅰ整合子阳性率占85.71%(48/56).结论:携带金属β-内酰胺酶铜绿假单胞菌在临床医院内广泛传播,需要加强院内感染监测及管理,并合理使用抗生素.
目的:瞭解本院臨床分離銅綠假單胞菌產生金屬β-內酰胺酶的耐藥現況與其耐藥基因攜帶率,併分析與亞胺培南耐藥關繫,為預防臨床感染及治療提供重要指導.方法:臨床收集2010-2012年患者標本併分離培養銅綠假單胞菌,菌株耐藥性採用紙片擴散法進行檢測,而金屬β-內酰胺酶採用雙紙片協同法檢測,運用聚閤酶鏈反應的方法檢測其耐藥基因.結果:在216株銅綠假單胞菌中檢測齣80株產金屬β-內酰胺酶,佔37.03%,其IMP與VIM基因檢齣率分彆為28.75%(23/80)與11.25%(9/80),其中9株攜帶VIM基因的菌株都對亞胺培南抗生素耐藥,而Ⅰ類整閤子檢齣率高達82.5%(66/80),其中亞胺培南耐藥菌株中Ⅰ整閤子暘性率佔85.71%(48/56).結論:攜帶金屬β-內酰胺酶銅綠假單胞菌在臨床醫院內廣汎傳播,需要加彊院內感染鑑測及管理,併閤理使用抗生素.
목적:료해본원림상분리동록가단포균산생금속β-내선알매적내약현황여기내약기인휴대솔,병분석여아알배남내약관계,위예방림상감염급치료제공중요지도.방법:림상수집2010-2012년환자표본병분리배양동록가단포균,균주내약성채용지편확산법진행검측,이금속β-내선알매채용쌍지편협동법검측,운용취합매련반응적방법검측기내약기인.결과:재216주동록가단포균중검측출80주산금속β-내선알매,점37.03%,기IMP여VIM기인검출솔분별위28.75%(23/80)여11.25%(9/80),기중9주휴대VIM기인적균주도대아알배남항생소내약,이Ⅰ류정합자검출솔고체82.5%(66/80),기중아알배남내약균주중Ⅰ정합자양성솔점85.71%(48/56).결론:휴대금속β-내선알매동록가단포균재림상의원내엄범전파,수요가강원내감염감측급관리,병합리사용항생소.
@@@@Objective:To survey the resistant of Metallo-β-lactamase and the distribution of genes in pseudomonas aeruginosa,and to analyze the relationship with imipenem-resisitant strain.so as to help carrying on the prevention and therapy of infection.Method:The clinical samples were collected from 2010 to 2012 in our hospital and the drug susceptibility test was measured by disk diffusion.The Metallo-β-lactamase of pseudomonas aeruginosa was detected by double disc synergy test.PCR was perferomed to amplify the resistant genes of imp vim spm gim and int1.Result:Among 216 strains, 80 strains were positive for MBL, the positive rate was 37.03%.IMP and VIM gene was found in 28.75%(23/80)and 11.25%(9/80) of the Metallo -β-lactamase-producing strain,The strains carrying VIM genes were resistance to imipenem. The positive rate of IntI1 was 82.5%(66/80),the IntI1 positive rate of Imipenem resistant strains was 85.71%(48/56).Conclusion:It is necessary to strengthened the nosocomial infection management and monitoring so as to preven MBL isolate from wide spread.
