目的探究分析鼻咽癌咽后淋巴结转移与EBV-DNA血清学标记物的关系.方法选取2005年1月-2012年8月于东莞市人民医院治疗NPC患者共100例,测量NPC咽后转移淋巴结的横截面积,横截面积<5 mm为阴性,横截面积≥5 mm为阳性.利用PCR技术测定血清EBV-DNA水平,根据试剂盒给定的标准:>5×102 copies/mL为阳性,否则为阴性,即拷贝数为0.结果此100例患者发生咽后部淋巴结转移者共有88例(88.0%),无淋巴结转移者有22例(22.0%),其EBV-DNA含量水平为0 copies/mL.其中单侧咽后部淋巴结转移者有54例(54.0%),其EBV-DNA含量水平为2.2×104 copies/mL;双侧咽后部淋巴结转移者有34例(34.0%),其EBV-DNA含量水平最高为1.1×105 copies/mL.经统计学检验证明:阴性组与单侧阳性EBV-DNA含量水平以及单侧阳性组与双侧阳性组EBV-DNA含量水平比较,差异有统计学意义(P<0.05);结论 EBV-DNA的检测可以作为CNP患者的诊断依据之一,其含量亦可以对CNP是否发生咽喉淋巴道转移的判断对其病情的严重程度以及预后的判断以及有指导意义,值得临床推广及应用.

Objective To explore the analysis of nasopharyngeal carcinoma lymph node metastasis and serological markers of EBV-DNA Methods NPC patients from January 2005 to August 2012 a total of 100 cases.Measurement NPC retropharyngeal lymph node metastases the cross-sectional area, the cross-sectional area of<5 mm negative cross-sectional area of≥5 mm positive. The determination of the level of serum EBV-DNA, using PCR technology according to the the kit given standard:>5 × 102 copies/mL positive or negative, i.e., the copy number is 0. Results This 100 patients retropharyngeal lymph node metastasis, a total of 88 cases (88.0%) without lymph node metastasis in 22 patients (22.0%), the levels of EBV-DNA 0 copies/mL. Unilateral retropharyngeal lymph node metastasis in 54 patients (54.0%), the levels of EBV-DNA 2.2 × 104 copies/mL;bilateral retropharyngeal lymph node metastasis in 34 patients (34.0%), the EBV-DNA The concentration levels of up to 1.1 × 105 copies/mL. The statistical test proved:negative group compared with the levels of EBV-DNA positive group with bilateral and unilateral positive EBV-DNA content level as well as unilateral positive group difference was statistically significant (P<0.05);Conclusion EBV-DNA detection can be used as one of the CNP patients diagnosis is based on the content of CNP occurred throat lymph node metastasis to determine the severity of their illness and prognosis and guiding significance worthy of promotion and application.
