目的:研究交感神经去除对胃内分泌细胞的影响,探讨降低迷走神经切除治疗十二指肠溃疡术后复发率的可能方法.方法:将30只Wistar大鼠随机分成2组,1组为模拟手术的对照组,2组行腹腔交感神经节切除,术后饲养6周后取材.测基础胃酸量、血清胃泌素水平,光镜下计胃粘膜中APUD细胞数、G细胞数、D细胞数,电镜下观察APUD细胞超微结构.结果:去除大鼠胃的交感神经后,(1)基础胃酸分泌量及血清胃泌素水平无明显变化;(2)胃的APUD细胞总数减少,G细胞减少,2组间有显著差异,D细胞无明显变化;(3) APUD细胞超微结构未见明显变化.结论:交感神经切除后,胃G细胞的减少可能是迷走神经合并交感神经去除后胃酸分泌恢复不全的原因之一.迷走神经切除治疗十二指肠溃疡时同时切除交感神经是降低术后复发率的可能方法.

목적:연구교감신경거제대위내분비세포적영향,탐토강저미주신경절제치료십이지장궤양술후복발솔적가능방법.방법:장30지Wistar대서수궤분성2조,1조위모의수술적대조조,2조행복강교감신경절절제,술후사양6주후취재.측기출위산량、혈청위비소수평,광경하계위점막중APUD세포수、G세포수、D세포수,전경하관찰APUD세포초미결구.결과:거제대서위적교감신경후,(1)기출위산분비량급혈청위비소수평무명현변화;(2)위적APUD세포총수감소,G세포감소,2조간유현저차이,D세포무명현변화;(3) APUD세포초미결구미견명현변화.결론:교감신경절제후,위G세포적감소가능시미주신경합병교감신경거제후위산분비회복불전적원인지일.미주신경절제치료십이지장궤양시동시절제교감신경시강저술후복발솔적가능방법.
Objective:Vagotomy has been performed for treating duodenal ulcer ,but it`s recurrence rateis very high,The gastric acid secretion always recovered completely to the preoperative level. But afterperformed vagotomy combined with sympathectomy,the gastric acid secretion didn`t recover completely. For reserch the probable reasons,we performed isolated sympathectomy on rat,the hanges of the basal gastricacid output and the serum gastrin and the amounts of APUD cells、G cells and D cells of rat gastric wereobserved. Methods:Thirty Wistar rats were divided into 2 groups in random,group 1 is control group, group 2 is operated group which were performed the extirpation of the coeliac and superior mesenteric ganglia. Six weeks after the operation,all animals were sampled,the basal gastric acid output and serumgastrin were measured,and the amounts of APUD cells、G cells and D cells were counted with a light microscope,the ultrastructure of APUD cells were observed with a transmission electron microscope. Results:(1) Sympathetic denervation of the stomach did not influence the basal gastric acid secretion and serum gastrin level.(2)The amounts of total APUD cells and G cells of rat gastric mucosa reduced significantly,and D cells had little changes.(3)The difference of ultrastructure of APUD cells between two groups had not been found. Conclusion:After sympathectomy,the reduction of the amount of total APUD cells and G cells of rat gastric mucosa had been observed,there was significantly statistical difference between two groups. This change may be one of the reasons of the uncompleted recovery of gastric acid output after vagotomy combined sympathectomy.
