目的探讨检测血浆脑钠肽和D-二聚体在冠心病诊断中的应用价值.方法选择109例经冠脉造影确诊的冠心病(CHD)患者且无严重肝肾功能损害,恶性肿瘤等并发症为CHD组,再根据临床分类分为急性心肌梗死(AMI)、不稳定型心绞痛(UAP)、稳定型心绞痛(SAP)三组,109例健康体检者为对照组,分别检测各组血浆中脑钠肽、D-二聚体浓度.结果 CHD组血浆脑钠肽、D-二聚体水平显著高于对照组(P<0.05);不同类型CHD组中,AMI组血浆脑钠肽、D-二聚体水平明显高于UAP组,UAP组血浆脑钠肽、D-二聚体水平明显高于SAP组,组间差异均有统计学意义(P<0.05).结论不同类型冠心病患者的血浆脑钠肽、D-二聚体水平存在差异,提示血浆脑钠肽、D-二聚体的水平有助于冠心病的早期诊断及反映其病变的严重程度.

@@@@Objective To Investigate the clinical significance of the plasma brain natriuretic peptide and d-dimer for the diagnosis to the coronary heart disease. Method To choose 109 definite CHD cases by coronary angiography, who had no liver/kidney damage、tumor and other diseases. They were to be three clinical classification: acute myocardial infarction(AMI), unstable angina pectoris(UAP), stable angina pectoris(SAP). The control group were 109 cases health examinations. Those were to detect plasma brain natriuretic peptide and d-dimer concentration. Result The plasma brain natriuretic peptide and d-dimer concentration of the CHD group were significantly higher than those of the control group(P<0.05), the level of brain natriuretic peptide and d-dimer in AMI group is obviously higher than those in UAP group, the level of brain natriuretic peptide and d-dimer in UAP group is obviously higher than those of the SAP group, the difference between groups were statistically significant(P<0.05). Conclusion The level of plasma brain natriuretic peptide and d-dimer are significantly increased in CHD, which are regarded as the CHD index for the diagnosis, treatment and prognosis.
