目的探讨多、微造瘘经皮肾镜钬激光治疗孤独肾复杂结石的疗效.方法分析自2008年5月至2012年8月我院13例孤独肾复杂性结石术中在B超联合X线下建立多、微造瘘,通道(14~18 F)置入输尿管硬镜F8/9用钬激光碎石取石,术后常规留置双“J”管及F14肾造瘘管.结果术中多通道穿刺目标肾盏进行碎石,手术时间97~113min,平均106min.术后:治愈10例,术后联合体外震波碎石治疗2例,术后感染性休克死亡1例.结论多、微造瘘经皮肾镜钬激光碎石取石是治疗孤独肾复杂结石具有创伤小、出血少、患者痛苦少、术后恢复快,结石清除率高等特点,值得临床推广.

Objective?To discuss multi-micro fistula percutaneous nephrolithotomy holmium laser in the treatment of solitary kidney complicated stone. Method From 2008 May to 2012 August analysis of 13 cases with solitary renal calculi combined with B ultrasound in complexity in X ray set, fistulization, channel (14~18F) placement of ureteroscopic holmium laser lithotripsy for F8/9 stone removal, routine postoperative indwelling double"J"pipe and F14 renal fistula. Result Intraoperative multichannel puncture target renal calyceal gravel, operation time is 97~113 minutes, with an average of 106 minutes. Postoperative:cure 10 cases, postoperative combined with extracorporeal shock wave lithotripsy for treatment of 2 cases of postoperative septic shock, 1 cases of death. Conclusions Multiple, micro fistula percutaneous renal ureteroscope and holmium:YAG laser lithotripsy and stone is the treatment of solitary renal complicated calculi with less trauma, less bleeding, less pain, faster postoperative recovery, the stone-free rate higher characteristic, is worth the clinical promoted.
