[目的]探讨益气养心汤联合传统西药治疗慢性心功能不全的疗效.[方法]30例慢性心功能不全患者在传统5大类西药强心、利尿、扩血管治疗基础上,加服益气养心汤,水煎服,1剂/d,2次/d,随症变化改变西药剂量;用药前及用药3个月后分别行6min步行实验评估心功能.[结果]服用中药前,30例患者6 min步行实验为100~410 m,平均(278.20±87.43)m;服用3个月后为220~620 m,平均(393.24±98.51)m,差异有统计学意义(P<0.05).在服用益气养心汤期间,患者在第1周内出现尿量增多情况,在第3周开始减少利尿剂和硝酸酯类药物的用量,期间均无明显水肿加重、小便减少情况,其中20例患者在2个月后停用硝酸酯类药物,25例患者将利尿剂减至最小剂量,5例患者停服各种利尿剂,随访期间心功能不全症状控制良好.[结论]益气养心汤安全可靠,副作用少,配合西医综合治疗可改善患者临床症状,降低对利尿剂和硝酸酯类药物的依赖性,不同程度提高活动耐量,尤其适用于心功能Ⅱ级和Ⅲ级的患者.
[목적]탐토익기양심탕연합전통서약치료만성심공능불전적료효.[방법]30례만성심공능불전환자재전통5대류서약강심、이뇨、확혈관치료기출상,가복익기양심탕,수전복,1제/d,2차/d,수증변화개변서약제량;용약전급용약3개월후분별행6min보행실험평고심공능.[결과]복용중약전,30례환자6 min보행실험위100~410 m,평균(278.20±87.43)m;복용3개월후위220~620 m,평균(393.24±98.51)m,차이유통계학의의(P<0.05).재복용익기양심탕기간,환자재제1주내출현뇨량증다정황,재제3주개시감소이뇨제화초산지류약물적용량,기간균무명현수종가중、소편감소정황,기중20례환자재2개월후정용초산지류약물,25례환자장이뇨제감지최소제량,5례환자정복각충이뇨제,수방기간심공능불전증상공제량호.[결론]익기양심탕안전가고,부작용소,배합서의종합치료가개선환자림상증상,강저대이뇨제화초산지류약물적의뢰성,불동정도제고활동내량,우기괄용우심공능Ⅱ급화Ⅲ급적환자.
Objective] To discuss the cure effect of Yiqi Yangxin Decoction combined with traditional western medicine(WN) on chronic dysfunctional heart. [Method] On the basis of traditional 6 big WM cardiotonic, diuresis and vascular dilation, 30 cases took Yiqi Yangxin Decoction, revising WM with symptoms;3m before and after medication, walking for 6min for evaluating heart function. [Result] Before medication, 30 cases took average(278.20±87.43) m for 6min walking;after medication for 3m, it was (393.24±98.51)m, the difference was of statistical meaning. During taking Yiqi Yangxin Decoction, the patients had more urine within first week, since 3th week, the diuretic and nitrate drugs were reduced, and there's no marked edema or reduced urine;but 20 cases stopped the nitrate and 25 decreased the diuretic to the minimum, 5 stopped al diuretic. Al cases were fol owed up with good heart dysfunctional con-trol. [Conclusion] Yiqi Yangxin Decoction is safe and reliable, with little side effects, together with WM, it can relieve clinical symptoms, reduce the depen-dence to diuretic and nitrate, to different extents improve activity tolerance, esp. fit for patients of heart function II and III levels.
