目的：分析医院病区不合理用药医嘱，为临床合理用药提供参考。方法运用合理用药监测系统及药师审核，对医院2014年1月至6月住院患者临床用药医嘱进行合理性审核并统计、分析。结果用药监测系统共监测到不合理医嘱15965条，其中以超剂量用药（55.78%）最常见，中药制剂的不合理使用也较多见。结论该院病区用药医嘱基本合理，但仍存在许多问题，需要临床医师和药师共同努力，提高临床合理用药水平。

목적：분석의원병구불합리용약의촉，위림상합리용약제공삼고。방법운용합리용약감측계통급약사심핵，대의원2014년1월지6월주원환자림상용약의촉진행합이성심핵병통계、분석。결과용약감측계통공감측도불합리의촉15965조，기중이초제량용약（55.78%）최상견，중약제제적불합리사용야교다견。결론해원병구용약의촉기본합리，단잉존재허다문제，수요림상의사화약사공동노력，제고림상합리용약수평。
Objective To analyze the irrational medical orders in the inpatient department of our hospital to provide reference for clini-cal rational drug use. Methods The clinical medication orders of the inpatient department in our hospital from January to June 2014 were performed the rationality check and the statistical analysis by using the rational drug use monitoring system and the pharmacist checking. Results 15 965 irrational medication orders were monitored by the medical monitoring system,in which the overdosage medi-cation was most common ( 55. 78% ) and the irrational use of the traditional Chinese medicine preparations also was more com-mon. Conclusion The medication orders of the inpatient department in our hospital are basically rational,but there are still many prob-lems,which need the joint effort of clinical physicians and pharmacists to increase the level of the clinical rational medication.
