目的：探讨谷氨酰胺(Gln)对合并胃肠功能障碍与衰竭(GIDF)的老年慢性阻塞性肺疾病急性加重期(AECOPD)患者的临床疗效。方法选择52例老年AECOPD合并GIDF患者，随机分为Gln 组(Gln辅助治疗组)26例和对照组26例，观察治疗后两组患者的胃肠功能恢复情况，血清前白蛋白、D-乳酸水平。结果Gln组患者在胃肠功能恢复、前白蛋白、D-乳酸水平方面与对照组比较，差异均有统计学意义(P<0.05)。结论合并GIDF的老年AECOPD患者补充谷氨酰胺可以促进患者胃肠功能恢复，改善营养状况，减轻炎症反应。
목적：탐토곡안선알(Gln)대합병위장공능장애여쇠갈(GIDF)적노년만성조새성폐질병급성가중기(AECOPD)환자적림상료효。방법선택52례노년AECOPD합병GIDF환자，수궤분위Gln 조(Gln보조치료조)26례화대조조26례，관찰치료후량조환자적위장공능회복정황，혈청전백단백、D-유산수평。결과Gln조환자재위장공능회복、전백단백、D-유산수평방면여대조조비교，차이균유통계학의의(P<0.05)。결론합병GIDF적노년AECOPD환자보충곡안선알가이촉진환자위장공능회복，개선영양상황，감경염증반응。
Objective To investigate the clinical efficacy of glutamine(Gln) in the treatment of elderly patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD) complicated with gastrointestinal dysfunction and failure(GIDF). Methods 52 AECOPD patients complicated with GIDF were randomly divided into two groups.The treatment group was additional y treated with Gln(n=26).Recovery of gastrointestinal function,concentration of serum prealbumin and D-lactate were compared between the two groups. Results The improvement was more pronounced in the treatment group than in the control group with significant difference(P<0.05). Conclusion Gln can promote the recovery of gastrointestinal function and reduce the inflammatory response in the treatment of patients with AECOPD complicated with GIDF.
