目的：探讨基层医院行微创经皮肾输尿管镜钬激光碎石术治疗上尿路结石的临床疗效。方法回顾分析总结微创经皮肾输尿管镜取石术治疗上尿路结石288例患者的临床资料。结果肾结石清除率95％，输尿管上段结石清除率100％，无胸膜损伤、气胸，无腹腔脏器损伤，无肾脏严重出血行肾切除病例，术后高热、感染8例，术后迟发出血5例，予以输血或超选择性肾动脉栓塞后治愈，死亡1例。结论微创经皮肾输尿管镜取石术治疗上尿路结石具有创伤小、出血少、并发症少等优点。
목적：탐토기층의원행미창경피신수뇨관경화격광쇄석술치료상뇨로결석적림상료효。방법회고분석총결미창경피신수뇨관경취석술치료상뇨로결석288례환자적림상자료。결과신결석청제솔95％，수뇨관상단결석청제솔100％，무흉막손상、기흉，무복강장기손상，무신장엄중출혈행신절제병례，술후고열、감염8례，술후지발출혈5례，여이수혈혹초선택성신동맥전새후치유，사망1례。결론미창경피신수뇨관경취석술치료상뇨로결석구유창상소、출혈소、병발증소등우점。
Objective To study the clinical effect of minimally invasive percutaneous nephrolithotomy ( MPCNL) with hol-mium laser in treatment of upper urinary stones in grass-roots hospitals.Methods Clinical data of 288 cases treated with MPCNL were summarized .Results The stone-free rate was 95%in patients with kidney stones and 100%for those with up-per urinary stones .No case was complicated with injury of pleura and aerothorax .No abdominal organ injury occurred .No case underwent renal hemorrhage nephrectomy .Urinary tract infection after operation was seen in 8 patients.5 patients complicated with delayed hemorrhage after operation were successfully treated with blood transfusion and super selective renal artery embo -lization.1 case died.Conclusion Treatment of upper urinary stones with MPCNL is characterized by small trauma ,less bleed-ing and fewer complications .It is worthy to be recommended in grass-roots hospitals .
