目的：探讨宫、腹腔镜在宫内节育器（ IUD）取出困难时的临床应用价值。方法回顾性分析2011年4月－2014年4月从外院转诊到我院或在我院门诊经常规取环失败、并经相关检查疑诊为IUD嵌顿、残留、断裂或异位的16例病例应用宫、腹腔镜诊断处理的临床资料。结果16例困难取环患者中，除1例宫外异位伴重度粘连中转开腹取环及另1例宫外异位伴脏器损伤直接剖腹取环外，余均经腹腔镜或宫腔镜或宫、腹腔镜联合而成功取出。结论宫、腹腔镜可明确困难环的具体状况并成功取出，具有创伤小、痛苦少、成功率高的优点，可作为宫内节育器取出困难时的首选措施。
목적：탐토궁、복강경재궁내절육기（ IUD）취출곤난시적림상응용개치。방법회고성분석2011년4월－2014년4월종외원전진도아원혹재아원문진경상규취배실패、병경상관검사의진위IUD감돈、잔류、단렬혹이위적16례병례응용궁、복강경진단처리적림상자료。결과16례곤난취배환자중，제1례궁외이위반중도점련중전개복취배급령1례궁외이위반장기손상직접부복취배외，여균경복강경혹궁강경혹궁、복강경연합이성공취출。결론궁、복강경가명학곤난배적구체상황병성공취출，구유창상소、통고소、성공솔고적우점，가작위궁내절육기취출곤난시적수선조시。
Objective To explore the value of clinical application of hysteroscopy and laparoscopy in difficult removal of intrauterine device(IUD).Methods From April 2011 to April 2014,16 patients diagnosed as inlay ,residue,fracture or ec-topic of IUD were enrolled .The data of treatment by hysteroscopy and laparoscopy were retrospectively analyzed .Results All the 16 patients were treated successfully by hysteroscopy ,laparoscopy ,or the combination ,except for 1 case of ectopic with se-vere adhesions who was turned to laparotomy to take the IUD out and 1 case of ectopic accompanied with organ injury who was treated with direct laparotomy to remove the IUD .Conclusion Hysteroscopy and laparoscopy can be used to take out IUD successfully , with small trauma , less pain and high rate of success .It can be used as the preferred measure ,and it is worthy of clinical application .
