目的:分析脑白质异常的语前聋儿人工耳蜗植入后听觉、言语、交流及智力的康复效果。方法:依照术前的影像学检查将语前聋的聋儿分为脑白质异常的人工耳蜗植入组(5例)和大脑发育正常的人工耳蜗植入组(5例),并配对组合,手术后一月,患儿开机后,采用汉语版人工耳蜗植入术后效果评估体系方法对两组患儿术后听觉能力、言语能力、交流能力及智力的康复效果进行评估分析,并用统计学方法比较两组患儿3个月与12个月时的患儿听觉、言语、交流及智力结果。结果:统计学分析显示术后3个月时,病例组的短句识别和声调识别的评估结果低于其配对儿,12个月后两组患儿的听力、言语、交流能力均无显著差异。结论:人工耳蜗植入术能够帮助伴有脑白质异常的重度感音神经性听力损失的患儿重建听力,不伴智力及躯体运动障碍的脑白质异常的聋儿应为人工耳蜗的适应症。
目的:分析腦白質異常的語前聾兒人工耳蝸植入後聽覺、言語、交流及智力的康複效果。方法:依照術前的影像學檢查將語前聾的聾兒分為腦白質異常的人工耳蝸植入組(5例)和大腦髮育正常的人工耳蝸植入組(5例),併配對組閤,手術後一月,患兒開機後,採用漢語版人工耳蝸植入術後效果評估體繫方法對兩組患兒術後聽覺能力、言語能力、交流能力及智力的康複效果進行評估分析,併用統計學方法比較兩組患兒3箇月與12箇月時的患兒聽覺、言語、交流及智力結果。結果:統計學分析顯示術後3箇月時,病例組的短句識彆和聲調識彆的評估結果低于其配對兒,12箇月後兩組患兒的聽力、言語、交流能力均無顯著差異。結論:人工耳蝸植入術能夠幫助伴有腦白質異常的重度感音神經性聽力損失的患兒重建聽力,不伴智力及軀體運動障礙的腦白質異常的聾兒應為人工耳蝸的適應癥。

Objective: With cochlear implant, to analysis the result of the hearing, speech, communication and intelli-gence in the preverbal deaf children with MRI abnormal white matter findings.Methods: A chinese treatment outcome evaluation system was used to evaluate the ability of the children with MRI abnormal white matter findings and the Prelingually deaf children with MRI nothing findings in auditory perception, Speech, communication, intelligence after 3 months implant and 1 year implant.Results: The discrimination scores of clause and tones in the case with MRI ab-normal white matter findings were found lower than the case with MRI nothing findings after 3 months implant. There were no statistical difference of the two groups in auditory perception, Speech, communication, intelligence after 12 months implant.Conclusion: Cochlear implantation could help the prelinguistic deaf children with MRI abnormal white matter findings to rebuild hearing. The deaf children with white matter abnormalities which not associated with mental disorders and somatic movement should be cochlear indications.
