Essential hypertension%N-terminal pro-brain natriuretic peptide (NT-proBNP)%Lymphocyte sub-population
目的:探讨原发性高血压(EH)患者N末端脑钠肽前体(NT-proBNP)及淋巴细胞亚群等指标变化情况。方法328例高血压确诊患者按照分级标准进行分组,其中EH1级组115例,EH2级组109例,EH3级组104例;以120例同期健康体检者作为对照组。采用电化学发光免疫分析测定NT-proBNP水平和流式细胞仪分析CD3+、CD4+、CD8+、CD19+水平,组间进行比较。结果高血压组血清中NT-proBNP、CD4+/CD8+、CD19+水平较健康对照组明显升高(P<0.05),其中EH3级组升高最显著;CD3+、CD4+、CD8+细胞明显低于正常对照组(P<0.05),EH3级组降低最显著。NT-proBNP水平与CD3+、CD4+、CD8+等指标均呈显著负相关(r=-0.951,r=-0.973,r=-0.955;P均<0.01),与CD19+细胞(B细胞)、CD4+/CD8+呈显著正相关(r=0.934,r=0.98;P均<0.01)。结论 EH患者的血压的临床分级与NT-proBNP、CD3+、CD4+、CD8+、CD19+的水平密切相关,并可能参与EH的病理生理过程。

목적:탐토원발성고혈압(EH)환자N말단뇌납태전체(NT-proBNP)급림파세포아군등지표변화정황。방법328례고혈압학진환자안조분급표준진행분조,기중EH1급조115례,EH2급조109례,EH3급조104례;이120례동기건강체검자작위대조조。채용전화학발광면역분석측정NT-proBNP수평화류식세포의분석CD3+、CD4+、CD8+、CD19+수평,조간진행비교。결과고혈압조혈청중NT-proBNP、CD4+/CD8+、CD19+수평교건강대조조명현승고(P<0.05),기중EH3급조승고최현저;CD3+、CD4+、CD8+세포명현저우정상대조조(P<0.05),EH3급조강저최현저。NT-proBNP수평여CD3+、CD4+、CD8+등지표균정현저부상관(r=-0.951,r=-0.973,r=-0.955;P균<0.01),여CD19+세포(B세포)、CD4+/CD8+정현저정상관(r=0.934,r=0.98;P균<0.01)。결론 EH환자적혈압적림상분급여NT-proBNP、CD3+、CD4+、CD8+、CD19+적수평밀절상관,병가능삼여EH적병리생리과정。
Objective To study the change of N-terminal pro-brain natriuretic peptide and lymphocyte sub-population in essential hypertensive patients. Methods Totally 328 hypertensive patients were divided into three groups according to the standard of classification:grade 1 hypertensive (115 patients), grade 2 hypertensive (109 pa-tients) and grade 3 hypertensive (104 patients). As the same time 120 normal controls were enrolled as control group. CD3+、CD4+、CD8+、CD19+level and serum NT-proBNP concentration were measured by flow cytometry and electroche-miluminescence immunoassay (ECLIA) . Results NT-ptoBNP, CD4/CD8 and CD19+ levels in 328 patients with EH were significantly higher than those in the controls (P<0.05) and they were increased with the increases of blood pres-sure, but CD3+, CD4+and CD8+levels were significantly lower (P<0.05) than the control and they were decreased with the increases of blood pressure. NT-proBNP level was negatively correlated with CD3+, CD4+ and CD8+ (r=-0.951, <br> r=-0.973, r=-0.955;P<0.01), and was positively correlated with CD19+ and CD4+/CD8+ (r=0.934, r=0.98; P<0.01). Conclusion NT-proBNP, CD3+, CD4+, CD8+ and CD19+were closely correlated with EH stage and probably in-volved in the pathophysiological process of EH.
