目的:探讨巨噬细胞移动抑制因子(MIF)与肿瘤坏死因子-α(TNF-α)水平在2型糖尿病患者中的变化及其与颈动脉粥样硬化、糖尿病肾病的相关性。方法随机选取本院内分泌科2011年5月至2013年10月收治的98例2型糖尿病患者,根据是否有颈动脉病变分为非颈动脉病变组(54例)与颈动脉病变组(44例),另选取同期体检健康者40例作为对照组,测定三组研究对象的血脂、肾功能等指标,比较并分析三组研究对象MIF、TNF-α、颈动脉内膜中层厚度(IMT)的差异及相关性。结果三组研究对象甘油三酯(TG)、总胆固醇(TC)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)水平比较差异无显著性(P>0.05);空腹血糖(FPG)、糖化血红蛋白(HbΑ1c)、MIF、TNF-α、IMT水平及尿清蛋白/肌酐(Α/C)比值比较差异具有显著性(P<0.05);非颈动脉病变组、颈动脉病变组患者FPG、HbΑ1c、MIF、TNF-α水平及Α/C比值显著高于对照组(P<0.05);颈动脉病变组患者IMT水平显著高于对照组(P<0.05);MIF水平与IMT水平、Α/C比值呈显著正相关(r=0.399,P=0.000;r=0.547,P=0.000);TNF-α水平与IMT水平、Α/C比值呈显著正相关(r=0.340, P=0.001;r=0.383,P=0.000)。结论 MIF与TNF-α在糖尿病颈动脉病变患者中表达水平较高,可能与颈动脉粥样硬化及糖尿病肾病具有一定相关性。

목적:탐토거서세포이동억제인자(MIF)여종류배사인자-α(TNF-α)수평재2형당뇨병환자중적변화급기여경동맥죽양경화、당뇨병신병적상관성。방법수궤선취본원내분비과2011년5월지2013년10월수치적98례2형당뇨병환자,근거시부유경동맥병변분위비경동맥병변조(54례)여경동맥병변조(44례),령선취동기체검건강자40례작위대조조,측정삼조연구대상적혈지、신공능등지표,비교병분석삼조연구대상MIF、TNF-α、경동맥내막중층후도(IMT)적차이급상관성。결과삼조연구대상감유삼지(TG)、총담고순(TC)、고밀도지단백담고순(HDL-C)、저밀도지단백담고순(LDL-C)수평비교차이무현저성(P>0.05);공복혈당(FPG)、당화혈홍단백(HbΑ1c)、MIF、TNF-α、IMT수평급뇨청단백/기항(Α/C)비치비교차이구유현저성(P<0.05);비경동맥병변조、경동맥병변조환자FPG、HbΑ1c、MIF、TNF-α수평급Α/C비치현저고우대조조(P<0.05);경동맥병변조환자IMT수평현저고우대조조(P<0.05);MIF수평여IMT수평、Α/C비치정현저정상관(r=0.399,P=0.000;r=0.547,P=0.000);TNF-α수평여IMT수평、Α/C비치정현저정상관(r=0.340, P=0.001;r=0.383,P=0.000)。결론 MIF여TNF-α재당뇨병경동맥병변환자중표체수평교고,가능여경동맥죽양경화급당뇨병신병구유일정상관성。
ObjectiveTo explore the correlation between changes in the expression level of macrophage migration inhibitory factor (MIF) and tumor necrosis factor-α (TNF-α) in type 2 diabetes patients and carotid atherosclerosis and diabetic nephropathy.Method90 patients with type 2 diabetes admitted into the department of endocrinology of our hospital from October 2011 to May 2013 were randomly selected, and they were divided into non carotid artery lesion group (54 patients) and carotid artery lesion group (44 patients) according to the existence of carotid artery, and 40 healthy person undergoing physical examination during the same period were separately selected. The blood lipid, renal function and other indexes of patients in three groups were determined, and the difference and correlation between MIF, TNF-α and carotid artery intima-media thickness (IMT) in three groups were compared and analyzed.ResultThe comparisons in levels of triglyceride (TG), total cholesterol (TC), high density lipoprotein cholesterol (HDL-C), low density lipoprotein cholesterol (LDL-C) of patients in three groups were not signiifcantly different (P>0.05), and the comparisons in levels of fasting blood glucose (FPG), glycosylated hemoglobin (HbΑ1c), MIF, TNF-α, IMT, urinary albumin creatinine (Α/C) of patients in three groups were signiifcantly different (P<0.05). The levels of FPG, HbΑ1c, MIF, TNF-α, Α/C of patients in non-carotid artery disease group and carotid artery disease groups were signiifcantly higher than those of patients in control group (P<0.05), and the level of IMT in patients in carotid artery lesions group was signiifcantly higher than that of control group (P<0.05). The level of MIF was positively related to MIF and A/C (r=0.399,P=0.000;r=0.547,P=0.000), and the level of TNF-α was positively related to IMT and Α/C (r=0.340,P=0.001;r=0.383,P=0.000). ConclusionThe expression levels of MIF and TNF-α were signiifcant in diabetic patients with carotid artery disease, and they are slightly related to carotid atherosclerosis and diabetic nephropathy.
