目的:探讨经阴道三维超声造影在输卵管源不孕症诊断中的应用。方法选取2012年10月~2013年12月本院生殖中心就诊疑患不孕症患者135例,采用经阴道三维超声造影诊断并治疗输卵管源不孕。结果超声造影通畅情况显示:92条输卵管通畅,107条输卵管通而不畅,59条输卵管阻塞。36例72条输卵管经3D-HyCoSy与腹腔镜通染液联合检查,结果显示两种方法具有较强的吻合度(κ=2.135,P<0.05)。以腹腔镜下输卵管通液检查结果为标准,经阴道3D-HyCoSy诊断输卵管通畅的阳性预测值为89.74%、阴性预测值为86.67%。结论经阴道子宫输卵管三维超声造影能真实地反映输卵管的空间走行、形态结构,是一种评价输卵管通畅性安全有效的检查方法。

목적:탐토경음도삼유초성조영재수란관원불잉증진단중적응용。방법선취2012년10월~2013년12월본원생식중심취진의환불잉증환자135례,채용경음도삼유초성조영진단병치료수란관원불잉。결과초성조영통창정황현시:92조수란관통창,107조수란관통이불창,59조수란관조새。36례72조수란관경3D-HyCoSy여복강경통염액연합검사,결과현시량충방법구유교강적문합도(κ=2.135,P<0.05)。이복강경하수란관통액검사결과위표준,경음도3D-HyCoSy진단수란관통창적양성예측치위89.74%、음성예측치위86.67%。결론경음도자궁수란관삼유초성조영능진실지반영수란관적공간주행、형태결구,시일충평개수란관통창성안전유효적검사방법。
ObjectiveTo explore the Transvaginal three-dimensional ultrasonography diagnosis of infertility in the application of the source in the fallopian.Methods 135 patients reproductivecenter clinic suspected in with infertility From 2012 October to 2013 year December in the hospitalwere all collected and who were all observed by using transvaginal three-dimensional ultrasonography in the diagnosis and treatment of tubal infertility source. ResultsUltrasoundangiography showed: tubal patency in 92, tubal and poor in 107, fallopian tube obstruction and 59. This group of 135 patients with 3D-HyCoSy and 36 cases of laparoscopic general stain combined examination of fallopian tube 72, and the Kappa coefficient test: two ways to fit and strong(κ=2.135,P<0.05). In order to laparoscopic tubal examination results as the standard, the positive predictive diagnosis of fallopian tube patency of the vaginal 3D-HyCoSy value, negative predictive value were 89.74%, 86.67%. Conclusion Transvaginal uterine tubal three-dimensional contrast-enhanced ultrasonography can reflect the transmission. Egg tube space walk, morphological structure, is a safe and effectiveevaluation of patency of fallopian tube inspection method.
