目的:探讨腰大池引流术在早期重型颅脑损伤中的临床应用效果。方法选取2010年8月~2014年6月在本院治疗重型颅脑损伤的64例患者作为研究对象,随机分为对照组和观察组,每组32例患者。对照组采用常规的腰穿放液治疗,观察组采用腰大池引流术进行治疗。比较两组患者在术后6个月内的预后情况及治疗前后的颅内压、脑血流动力学指标。结果观察组治疗后的预后情况优于对照组患者,术后并发症均低于对照组患者,差异均有统计学意义(P<0.05)。结论腰池持续引流治疗早期重型颅脑损伤的疗效较佳,可显著改善患者的颅内压及脑血流状态,促进患者的预后,值得临床推广应用。

목적:탐토요대지인류술재조기중형로뇌손상중적림상응용효과。방법선취2010년8월~2014년6월재본원치료중형로뇌손상적64례환자작위연구대상,수궤분위대조조화관찰조,매조32례환자。대조조채용상규적요천방액치료,관찰조채용요대지인류술진행치료。비교량조환자재술후6개월내적예후정황급치료전후적로내압、뇌혈류동역학지표。결과관찰조치료후적예후정황우우대조조환자,술후병발증균저우대조조환자,차이균유통계학의의(P<0.05)。결론요지지속인류치료조기중형로뇌손상적료효교가,가현저개선환자적로내압급뇌혈류상태,촉진환자적예후,치득림상추엄응용。
Objective To investigate the clinical application effect of lumbar cistern drainage insevere craniocerebral injury in early stage.MethodsWe selectd 64 cases of patients who were treated for severe craniocerebral injury in our hospital from August 2010 to June 2014 as the research object, they were randomly divided into two groups, the control group and the observation group, each group had 32 cases. The control group was treated with routine lumbar puncture drainage, the observation group was treated with lumbar cistern drainage. The outcomes of the two groups were compared within 6 months after operation and before and after the treatment of intracranial pressure, cerebral hemodynamics.Results The prognosis of the observation group was better than that of the control group, the postoperative complications were lower than the patients in the control group, the difference was statistically significant (P<0.05).Conclusion Continuous lumbar cistern drainage for the treatment of early severe craniocerebral injury is relatively better, it can significantly improve the state of patients with intracranial pressure and cerebral blood flow, promote the prognosis of the patients, it is worthy for clinical application.
