With the profound understanding of ventilator-associated lung injury in acute respiratory distress syndrome, high-frequency oscillatory ventilation is considered to be an ideal ventilation strategy. There is still a big controversy in the clinical application of high-frequency oscillatory ventilation in adult patients with acute respiratory distress syndrome because of some defects, especially the results of two latest large multicenter randomized controlled trials were disappointing. However, the mechanism, technology, ventilation strategies, and monitoring system of high-frequency oscillatory ventilation need to be further explored .
