目的探讨对居家糖尿病患者实施结构性血糖监测的具体措施和管理方法,提高糖尿病患者自我血糖监测效果。方法对102例糖尿病患者实施居家结构性血糖监测6个月,内容包括实施结构性血糖监测的方法、血糖值曲线图的绘制与医患共同解读等关键环节与管理要点,观察实施结构性血糖监测前及管理6个月后患者的空腹血糖,餐后2小时血糖,糖化血红蛋白及评估患者自我血糖监测的依从性。结果实施糖尿病患者居家结构性血糖监测和管理6个月后患者的FBG、PBG、HbA1c水平均低于实施前,差异具有统计学意义;患者自我血糖监测的依从性明显提高,差异具有统计学意义。结论实施糖尿病患者居家结构性血糖监测与强化管理能提高患者自我血糖监测效果,提供个性化血糖谱分析医患互动解读并及时调整治疗方案,使结构性自我血糖监测成为有效的糖尿病患者自我管理的工具。

Objective Explore specific measures and management methods for structural monitoring of blood glucose (SMBG) on diabetic patients at home and their improve self-monitoring blood glucose effects. Methods Implemented structural monitoring of blood glucose on 102 patients for six months. Implementation content included its method, blood glucose graph drawing and other key aspects of common interpretation by doctor and patient as well as management. Observed the patient's fasting blood glucose (FBG), 2h postprandial blood glucose (PBG), glycosylated hemoglobin (HbA1c) and assessed the compliance of patient self-monitoring of blood glucose in the implementation period. Results After structural blood glucose monitoring and management for six months, the patients' FBG, PBG and HbA1c levels were lower than before (P<0.005). Patients' compliance of self-monitoring of blood glucose significantly increased (P<0.005). Conclusion Implementing of structural monitoring of blood glucose in diabetic patients at home and strengthening management can improve patients' self-monitoring of blood glucose results. It can provide personalized blood glucose spectrum analysis and doctor-patient interaction, which help adjust the treatment plan and make the SMBG become effective self-management tools for diabetes patients.
