目的:探讨经脉络膜裂入三脑室手术入路的显微解剖学研究,并为最大程度降低正常脑组织损伤提供理论依据。方法8例经10%福马林固定的国人成人头颅湿性标本,动静脉系统各自灌注含有红蓝染料乳胶;使用可放大4~10倍的手术显微镜,对标本侧脑室脉络裂部分按照手术入路途径逐层解剖并观察。结果脉络膜裂处于穹隆与丘脑间,呈现倒“C”形状。分为(1)体部:处在侧脑室体部中,丘脑内上缘与穹隆体之间;(2)三角部:处在侧脑室三角区中,丘脑枕与穹隆脚间;(3)颞部:处在侧脑室颞角中,丘脑下表面与穹隆伞间。结论经脉络膜裂入三脑室手术入路对第三脑室、环池、颞叶内侧的病变进行处理,具有术野暴露好、手术损伤小等优点。

목적:탐토경맥락막렬입삼뇌실수술입로적현미해부학연구,병위최대정도강저정상뇌조직손상제공이론의거。방법8례경10%복마림고정적국인성인두로습성표본,동정맥계통각자관주함유홍람염료유효;사용가방대4~10배적수술현미경,대표본측뇌실맥락렬부분안조수술입로도경축층해부병관찰。결과맥락막렬처우궁륭여구뇌간,정현도“C”형상。분위(1)체부:처재측뇌실체부중,구뇌내상연여궁륭체지간;(2)삼각부:처재측뇌실삼각구중,구뇌침여궁륭각간;(3)섭부:처재측뇌실섭각중,구뇌하표면여궁륭산간。결론경맥락막렬입삼뇌실수술입로대제삼뇌실、배지、섭협내측적병변진행처리,구유술야폭로호、수술손상소등우점。
Objective To investigate the microscopic anatomy study of surgical approach into the third ventricle through the cho-roid fissure and to provide the theoretical basis for minimizing normal brain tissue damage. Methods 8 cases of Chinese adult moist skull specimens were fixed with 10% formalin, and each arterial and venous system were poured with red and blue dye latex. The cho-roid fissure part of lateral ventricle in specimen was dissected and observed layer by layer according to the surgical approach by using 4-10 time zoom surgical microscope. Results Choroid fissure was an inverted “C” shape between the dome and the hypothalamus, and it can be divided into (1) The body:located in the body of lateral cerebral ventricle between the upper margin of the thalamus and the dome body. (2) Triangular part:in the triangle area in the lateral ventricles between the pulvinar and dome foot. (3) Temporal:in the temporal horn of the lateral ventricle between the surface and the dome umbrella hypothalamus. Conclusion Surgical approach through choroid fissure into the third ventricle, the central pool and medial temporal lobe lesions bears the advantage of wider operative horizon, less surgical injury and so on.
