目的:研究CT引导下经皮肺穿刺切割活检术的临床应用和并发症的分析。方法总结分析2011年12月至2014年7月间,经皮肺活检术的120例的临床资料,分析诊断价值及患者穿刺次数、肿瘤边缘和胸膜的距离、病灶大小、穿刺时间对气胸和肺内出血发生率的相关性研究。结果120例均穿刺成功,获取组织经病理和免疫组化确诊116例,确诊率96.7%。气胸和/或出血23.3%(28/120),气胸发生率为16.7%(20/120);出血发生率为13.3%(16/120);其中需要胸腔引流的气胸为2.5%(3/120),无死亡病例。结论该操作简单易行,损伤较小,风险较低,穿刺成功率及确诊率均较高,对确诊肺内占位性病变,具有重要价值。

목적:연구CT인도하경피폐천자절할활검술적림상응용화병발증적분석。방법총결분석2011년12월지2014년7월간,경피폐활검술적120례적림상자료,분석진단개치급환자천자차수、종류변연화흉막적거리、병조대소、천자시간대기흉화폐내출혈발생솔적상관성연구。결과120례균천자성공,획취조직경병리화면역조화학진116례,학진솔96.7%。기흉화/혹출혈23.3%(28/120),기흉발생솔위16.7%(20/120);출혈발생솔위13.3%(16/120);기중수요흉강인류적기흉위2.5%(3/120),무사망병례。결론해조작간단역행,손상교소,풍험교저,천자성공솔급학진솔균교고,대학진폐내점위성병변,구유중요개치。
Objective: to analyze the clinical application and complications of CT guided percutaneous lung puncture biopsy. Methods: to ana-lyze and summarize the 2011 December to 2014 July, clinical data of 120 patients with the percutaneous lung biopsy, to evaluate the diag-nostic value and number of puncture, the edge of tumor patients and pleural lesion size, distance, time of puncture of pneumothorax and in-trapulmonary hemorrhage correlation between incidence rate. Results:120 cases were successful puncture, obtaining tissue pathology and im-munohistochemistry confirmed 116 cases, diagnosis rate 96.7%. Pneumothorax and / or hemorrhage in 23.3% (28/120), the incidence of pneumothorax was 16.7% (20/120); bleeding rate was 13.3% (16/120); the need of thoracic drainage pneumothorax was 2.5% (3/120), no death case. Conclusion:the operation is simple, little injury, lower risk, puncture success rate and diagnosis rate is higher, to the diagnosis of pulmonary lesions, which has important value.
