目的:探讨运用授权原理及多阶段改变理论结合的教育模式干预老年糖尿病患者饮食行为对患者代谢指标、饮食行为及授权能力的影响。方法118例老年2型糖尿病患者,随机分成两组。对照组55例,采用传统的教育方法;干预组63例,采用授权原理及多阶段改变理论相结合的教育方法。比较两组患者血糖、血脂、中文版糖尿病授权简化量表( C-DES-SF )评分、多阶段行为改变阶段。结果干预组空腹血糖、餐后2 h血糖、总胆固醇、低密度脂蛋白改善程度好于对照组( P<0.05),干预后,干预组患者C-DES-SF评分高于对照组(P<0.05),处于行动阶段及维持阶段的患者比例高于对照组(P<0.05)。结论运用授权原理及多阶段改变理论相结合的教育模式对老年糖尿病患者进行教育能帮助患者改善血糖、血脂指标,促进患者饮食行为改变及授权能力的提高。

목적:탐토운용수권원리급다계단개변이론결합적교육모식간예노년당뇨병환자음식행위대환자대사지표、음식행위급수권능력적영향。방법118례노년2형당뇨병환자,수궤분성량조。대조조55례,채용전통적교육방법;간예조63례,채용수권원리급다계단개변이론상결합적교육방법。비교량조환자혈당、혈지、중문판당뇨병수권간화량표( C-DES-SF )평분、다계단행위개변계단。결과간예조공복혈당、찬후2 h혈당、총담고순、저밀도지단백개선정도호우대조조( P<0.05),간예후,간예조환자C-DES-SF평분고우대조조(P<0.05),처우행동계단급유지계단적환자비례고우대조조(P<0.05)。결론운용수권원리급다계단개변이론상결합적교육모식대노년당뇨병환자진행교육능방조환자개선혈당、혈지지표,촉진환자음식행위개변급수권능력적제고。
Objective To investigate the effects of empowerment and stages of change education model applied to diet intervention on patients′metabolic index ,diet behavior and empowerment ability in elderly patients with diabetes mellitus .Methods One hundred and eighteen elderly patients with type 2 diabetes mellitus were randomly divided into two groups .Fifty-five cases in the control group were educated with the traditional method ,63 cases in the intervention group were educated with the empowerment and stages of change education model .Patients′blood glucose , blood lipid,Chinese diabetes empowerment scale-short form(C-DES-SF) score and multi-stage behavior change stage were compared between two groups .Results The improvement of fasting blood glucose ,2 hour postprandial blood glucose ,total cholesterol,low-density lipoprotein cholesterol in the intervention group was better than that in the control group (P<0.05). After the intervention ,the score of C-DES-SF in the intervention group was higher than that in the control group ( P<0.05) , and the proportion of patients in the stage of action and maintain was higher in the intervention group compared with control group(P<0.05).Conclusion It can improve patient′s blood glucose and blood lipid , promote their diet behavior changes and improve their empowerment ability by applying the empowerment and stages of change education model to diet intervention on elderly patients with diabetes mellitus .
