目的:探讨骨折延迟愈合患者血清可溶性细胞间黏附分子( sICAM-1)、血管细胞黏附分子(VCAM-1)、骨形态发生蛋白2(BMP-2)及微循环指标的变化情况。方法50例骨折延迟愈合(A组),50例骨折正常愈合者( B组),50例健康志愿者( C组),检测3组血清sICAM-1、VCAM-1、BMP-2水平及红细胞聚集、血液流变学指标。结果 A 组血清sICAM-1、VCAM-1均高于B 组及C 组,而血清BMP-2则低于B 组、C 组(P<0.05);A组的红细胞聚集程度低于B组及C组(P<0.05);A组的全血低切黏度、血浆黏度、红细胞电泳指数均高于B组及C组(P均<0.05)。结论骨折延迟愈合患者血清sICAM-1、VCAM-1水平升高,BMP-2水平降低,并且微循环状态较差,积极干预可避免或者降低高危人群发生骨折延迟愈合的风险。

목적:탐토골절연지유합환자혈청가용성세포간점부분자( sICAM-1)、혈관세포점부분자(VCAM-1)、골형태발생단백2(BMP-2)급미순배지표적변화정황。방법50례골절연지유합(A조),50례골절정상유합자( B조),50례건강지원자( C조),검측3조혈청sICAM-1、VCAM-1、BMP-2수평급홍세포취집、혈액류변학지표。결과 A 조혈청sICAM-1、VCAM-1균고우B 조급C 조,이혈청BMP-2칙저우B 조、C 조(P<0.05);A조적홍세포취집정도저우B조급C조(P<0.05);A조적전혈저절점도、혈장점도、홍세포전영지수균고우B조급C조(P균<0.05)。결론골절연지유합환자혈청sICAM-1、VCAM-1수평승고,BMP-2수평강저,병차미순배상태교차,적겁간예가피면혹자강저고위인군발생골절연지유합적풍험。
Objective To investigate the changes in serum soluble intercellular adhesion molecule-1(sICAM-1), vascular cell adhesion molecule-1(VCAM-1),bone morphogenetic protein-2(BMP-2) and microcirculation indexes of patients with delayed fracture healing .Methods Fifty patients with delayed fracture healing ( group A ) ,50 patients with normal fracture healing ( group B ) , and 50 healthy people ( group C ) were enrolled in the study .The serum sICAM-1,sVCAM-1,BMP-2 and the indexes of erythrocyte aggregation and hemorheology were detected among three groups .Results The serum levels of sICAM-1 and VCAM-1 in group A were higher than those of group B and group C , but the serum BMP-2 level was lower than those of group B and group C ( P<0.05) .The erythrocyte aggregation rate in group A was lower than those of group B and group C ( P <0.05 ) .The whole blood viscosity , plasma viscosity and erythrocyte electrophoresis index in group A were higer than those of group B and group C (all P<0.05).Conclusion The serum sICAM-1 and VCAM-1 levels increase but BMP-2 level decreases in patients with delayed fracture healing , with worse microcirculation .Active intervention could benefit avoiding or decreasing the risk of delayed fracture healing in high-risk population .
