目的:探讨肌萎缩侧索硬化症(ALS)在3.0T MRI 及弥散张量纤维束示踪技术(DTT)上的影像表现.方法:使用 GE 3.0T MR,对病例组进行 T1WI,T2WI,FLAIR 及 DTI 扫描,分析其在常规 MRI 上的表现;进行 DTT 成像,分析纤维束的形态及连续性.结果:69例病例组中,脑萎缩患者35例,占50.7%;在 FLAIR 及 T2WI 上皮质脊髓束走行区高信号患者,27例,占39.1%;中央前回皮质下“黑线征”为21例,占30.2%;69例病例组皮质脊髓束在三维彩色编码张量图上颜色混杂,在弥散张量纤维束示踪图上呈 FA 值较低的绿色或蓝色;部分病例组(9例)双侧皮质脊髓束形态欠对称;部分病例组(12例)脑桥层面未见纤维束通过.结论:ALS 患者的常规 MRI 表现阳性率低;三维彩色编码张量图通过三种颜色混杂提示纤维束走行方向紊乱;弥散张量纤维束示踪图可直观显示 ALS 患者纤维束的损害.

목적:탐토기위축측색경화증(ALS)재3.0T MRI 급미산장량섬유속시종기술(DTT)상적영상표현.방법:사용 GE 3.0T MR,대병례조진행 T1WI,T2WI,FLAIR 급 DTI 소묘,분석기재상규 MRI 상적표현;진행 DTT 성상,분석섬유속적형태급련속성.결과:69례병례조중,뇌위축환자35례,점50.7%;재 FLAIR 급 T2WI 상피질척수속주행구고신호환자,27례,점39.1%;중앙전회피질하“흑선정”위21례,점30.2%;69례병례조피질척수속재삼유채색편마장량도상안색혼잡,재미산장량섬유속시종도상정 FA 치교저적록색혹람색;부분병례조(9례)쌍측피질척수속형태흠대칭;부분병례조(12례)뇌교층면미견섬유속통과.결론:ALS 환자적상규 MRI 표현양성솔저;삼유채색편마장량도통과삼충안색혼잡제시섬유속주행방향문란;미산장량섬유속시종도가직관현시 ALS 환자섬유속적손해.
Aim:To investigate the manifestation of amyotrophic lateral sclerosis(ALS)on 3.0T MRI and diffusion tensor tracking(DTT).Methods:All subjects were performed axial T1WI,T2WI,FLAIR and diffusion tensor imaging(DTI)with GE 3.0T MRI,and were analyzed the manifestation on conven-tional MRI,the shape and continuity of corticospinal tract(CST)on DTT were analyzed.Results:There were 35 cases of brain atrophy,which was 50.7%,27 cases on the hyperintense signal on FLAIR and T2WI,which was 39.1%,while it was 21 cases of linar hypointense signal along precentral gyrus on FLAIR,which was 30.2%.The CST fiber showed mixed colors on directionally encoded color(DEC), with green or blue color which was low fractional anisotropy on DTT,The CST shape was asymmetric on partial patients while there was on fiber to pons on partial cases.Conclusion:The positive rate of ALS patients is low on conventional MRI;the mixed color On DEC shows the disorder of fiber,DTT can show the damage of CST directly.
