目的：了解医院门、急诊处方的药物使用情况，以减少药品不良反应发生率，促进合理用药。方法：随机抽取2013年每月门、急诊处方400张，共计4800张，根据《医院处方点评管理规范（试行）》进行点评、分析。结果：4800张处方中，不合理处方248张；不合理用药主要表现为用药与临床诊断不相符、用法与用量不适宜、联合用药不适宜、有配伍禁忌、溶剂使用不当等。结论：应定期进行处方点评，分析临床用药存在的问题，及时发现、合理干预，以提高临床合理用药水平。
목적：료해의원문、급진처방적약물사용정황，이감소약품불량반응발생솔，촉진합리용약。방법：수궤추취2013년매월문、급진처방400장，공계4800장，근거《의원처방점평관리규범（시행）》진행점평、분석。결과：4800장처방중，불합리처방248장；불합리용약주요표현위용약여림상진단불상부、용법여용량불괄의、연합용약불괄의、유배오금기、용제사용불당등。결론：응정기진행처방점평，분석림상용약존재적문제，급시발현、합리간예，이제고림상합리용약수평。
OBJECTIVE:To investigate the drug use in the outpatient and emergency department so as to reduce the incidence of adverse drug reactions and promote rational use of drugs. METHODS: 400 outpatient & emergency prescriptions collected monthly ( 4 800 prescriptions in total ) were reviewed in accordance with to the “Management Practices on Hospital Prescription Review( Trial)”. RESULTS:248 outpatient prescriptions were unqualified, and the irrational use of drugs manifested as incompatibility between drug use and the clinical diagnosis, inappropriate in usage& dosage or drug combination, incompatibility and improper use of solvents etc. CONCLUSIONS: Regular review on prescriptions and analysis of the problems in clinical drug use and timely intervention can help improve clinicians?level in rational use of drugs.
