目的:观察红外线联合308nm准分子激光照射皮损局部治疗白癜风的疗效,并分析其对表皮黑素细胞内α-MSH的含量的影响。方法:随机选取我院皮肤科门诊2012年3月~2014年3月的稳定期白癜风患者60例,将全部患者平均分为3组,即对照组A、对照组B和实验组,每组患者有20例。对照组A单纯采用红外线照射治疗;对照组B单纯采用308nm准分子激光治疗;实验组采用红外线联合308 nm准分子激光治疗。3组患者均选取1处躯干部位的皮损,分别采用各自的治疗方案,皮损部位均每周照射2次,每月为1疗程,连续3个疗程后对临床效果进行评价,并分析对黑素细胞α-MSH分泌的影响。结果:对照组A和对照组B的总有效率为分别为45.0%和50.0%,实验组总有效率为70.0%,实验组与对照组A与对照组B相比总有效率差异显著,具有统计学意义,P<0.05。实验组α-MSH含量与对照组A和对照组B相比,差异显著,P<0.05;对照组A和对照组Bα-MSH含量相比差异无显著性,P>0.05。结论:红外线联合308nm准分子激光可增加黑素细胞中α-MSH的分泌,促进黑素含量增加、局部色素沉着,从而提升白癜风治疗效果,疗效确切,安全可靠,提升了患者的生活质量,增加了社会效益和经济效益。

목적:관찰홍외선연합308nm준분자격광조사피손국부치료백전풍적료효,병분석기대표피흑소세포내α-MSH적함량적영향。방법:수궤선취아원피부과문진2012년3월~2014년3월적은정기백전풍환자60례,장전부환자평균분위3조,즉대조조A、대조조B화실험조,매조환자유20례。대조조A단순채용홍외선조사치료;대조조B단순채용308nm준분자격광치료;실험조채용홍외선연합308 nm준분자격광치료。3조환자균선취1처구간부위적피손,분별채용각자적치료방안,피손부위균매주조사2차,매월위1료정,련속3개료정후대림상효과진행평개,병분석대흑소세포α-MSH분비적영향。결과:대조조A화대조조B적총유효솔위분별위45.0%화50.0%,실험조총유효솔위70.0%,실험조여대조조A여대조조B상비총유효솔차이현저,구유통계학의의,P<0.05。실험조α-MSH함량여대조조A화대조조B상비,차이현저,P<0.05;대조조A화대조조Bα-MSH함량상비차이무현저성,P>0.05。결론:홍외선연합308nm준분자격광가증가흑소세포중α-MSH적분비,촉진흑소함량증가、국부색소침착,종이제승백전풍치료효과,료효학절,안전가고,제승료환자적생활질량,증가료사회효익화경제효익。
Objective:To observe the clinical effect of the treatment of leucroderma impaired skin by infrared ray combined with 308 nm excimer laser, and to analyze its influence on the amount ofα-MSH in epidermal melanocytes.Mehtods:Sixty leucroderma patients at stable phase in treatment in dermatology department of our hospital between February, 2012 and March, 2014 were randomly selected as participants, and were divided into three groups, namely, control group A, control group B and treatment group, with 20 patients in each group.For patients in control group A, infrared radiation therapy was adopted;for patients in control group B, 308nm excimer laser thera-py was adopted;for patients in treatment group A, therapy of infrared ray combined with 308nm excimer laser was adopted.For patients in each group, an impaired part of trunk skin was treated with respective therapy, twice a week, one month a treatment course.The clini-cal effects were evaluated and the influence ofα-MSH secretion in melanocytes was analyzed after three successive treatment courses.Re-sults:The total effective rates of control group A and control group B was respectively 45.0%and 50.0%, and that of the treatment group was 70.0%.The differences between treatment group and control groups A and B were statistically significant, with P<0.05;the differ-ences in the amount ofα-MSH between treatment group and control groups A and B were statistically significant, with P<0.05; the difference in the amount ofα-MSH between control group A and control group B was statistically insignificant, with P>0.05.Conclu-sion:The combination therapy of infrared ray and 308nm excimer laser can increase the secretion ofα-MSH in melanocytes in leucroder-ma patients, and spur the content of melanine, local skin pigmentation, and thus promote the treatment effects of leucroderma, which is definite, safe, and sure.Therefore, this therapy can promote the living quality of patients and increase social and economic benefits.
