目的:总结及探讨应用腓肠神经营养血管筋膜皮瓣修复足部皮肤软组织缺损的疗效与经验。方法2007年3月—2014年3月,应用腓肠神经营养血管筋膜皮瓣修复合并肌腱和(或)骨外露的足部皮肤与软组织缺损108例,皮瓣面积:8.0 cm×15.0 cm~16.0 cm×27.0 cm。供区直接缝合27例,其余采用游离植皮修复。结果108例皮瓣移植104例全部成活,4例皮瓣远端部分坏死,2例经扩创换药后直接愈合,2例经再次植皮修复创面愈合。经3~18个月随访,皮瓣质地柔软,弹性好,无因皮瓣导致的足部功能障碍。结论采用腓肠神经营养血管筋膜皮瓣修复足部皮肤软组织缺损,皮瓣成活率高,手术操作简单,不损伤主干血管,是恢复足部功能的有效方法。
目的:總結及探討應用腓腸神經營養血管觔膜皮瓣脩複足部皮膚軟組織缺損的療效與經驗。方法2007年3月—2014年3月,應用腓腸神經營養血管觔膜皮瓣脩複閤併肌腱和(或)骨外露的足部皮膚與軟組織缺損108例,皮瓣麵積:8.0 cm×15.0 cm~16.0 cm×27.0 cm。供區直接縫閤27例,其餘採用遊離植皮脩複。結果108例皮瓣移植104例全部成活,4例皮瓣遠耑部分壞死,2例經擴創換藥後直接愈閤,2例經再次植皮脩複創麵愈閤。經3~18箇月隨訪,皮瓣質地柔軟,彈性好,無因皮瓣導緻的足部功能障礙。結論採用腓腸神經營養血管觔膜皮瓣脩複足部皮膚軟組織缺損,皮瓣成活率高,手術操作簡單,不損傷主榦血管,是恢複足部功能的有效方法。
목적:총결급탐토응용비장신경영양혈관근막피판수복족부피부연조직결손적료효여경험。방법2007년3월—2014년3월,응용비장신경영양혈관근막피판수복합병기건화(혹)골외로적족부피부여연조직결손108례,피판면적:8.0 cm×15.0 cm~16.0 cm×27.0 cm。공구직접봉합27례,기여채용유리식피수복。결과108례피판이식104례전부성활,4례피판원단부분배사,2례경확창환약후직접유합,2례경재차식피수복창면유합。경3~18개월수방,피판질지유연,탄성호,무인피판도치적족부공능장애。결론채용비장신경영양혈관근막피판수복족부피부연조직결손,피판성활솔고,수술조작간단,불손상주간혈관,시회복족부공능적유효방법。
Objective To summary and discuss the curative effect and experience of repairing soft tissue defects of the foot by application of sural neurovascular fasciocutaneous flap. Methods From March 2007 to March 2014, skin and soft tissue defects combined tendon and (or) bone exposed on the foot were repaired by sural neurovascular fasciocutaneous flap in 108 cases. The size of flap was 8.0 cm × 15.0 cm to 16.0 cm × 27.0 cm. The donor sites were sutured directly in 27 cases, others were repaired by skin graft. Results 104 cases survived completely, distal of flap was necrosis in 4 cases, the wounds were healed by dressing changing in 2 cases, and by skin grafting in 2 cases. After 3~18 months of follow-up, the flaps showed soft texture and good elasticity. Conclusion It is a simple, high survival rate, no major vascular injury method that by using sural neurovascular fasciocutaneous flap to repair the soft tissue defects of the foot.
