目的：探讨术前检测D－二聚体（D －dimer ，DD）在预防妇科盆腔手术后静脉血栓形成中的作用。方法：收集2003年1月至2009年9月乌海市妇幼保健院妇科收治的盆腔手术病例，共608例，随机分为术前检测 DD 组（238例）和对照组（370例）。其中，术前检测DD组中DD浓度大于500 ng／mL的患者术后第二天开始给予皮下注射低分子肝素钠12500U／d，共7d，同时口服阿司匹林75mg／d；对照组术后未应用抗凝剂。两组均随访观察至术后6个月，比较两组静脉血栓的发生率。结果：检测组术后6个月无形成静脉血栓者，对照组术后有9例患者形成下肢静脉血栓，经比较差异有统计学意义（ P ＜0．05）。结论：术前检测DD ，可指导术后是否应用抗凝剂，从而有效预防术后静脉血栓的形成。
목적：탐토술전검측D－이취체（D －dimer ，DD）재예방부과분강수술후정맥혈전형성중적작용。방법：수집2003년1월지2009년9월오해시부유보건원부과수치적분강수술병례，공608례，수궤분위술전검측 DD 조（238례）화대조조（370례）。기중，술전검측DD조중DD농도대우500 ng／mL적환자술후제이천개시급여피하주사저분자간소납12500U／d，공7d，동시구복아사필림75mg／d；대조조술후미응용항응제。량조균수방관찰지술후6개월，비교량조정맥혈전적발생솔。결과：검측조술후6개월무형성정맥혈전자，대조조술후유9례환자형성하지정맥혈전，경비교차이유통계학의의（ P ＜0．05）。결론：술전검측DD ，가지도술후시부응용항응제，종이유효예방술후정맥혈전적형성。
Objective :To explore the role of preoperative D -dimer (DD) detection in preventing the venous thrombo_sis after gynecological pelvic surgery .Methods :608 gynecological pelvic surgery patients hospitalized and treated in the Wuhai Hospital of Maternal and Child Health from January 2003 to September 2009 were randomly divided into the pre_operative DD detection group (238cases) and the control group (370 cases) .In the preoperative DD detection group ,the patients of DD values in plasma over 500ng/ml were given subcutaneous injection of low molecular weight heparin sodium 12500u/day together with oral administration of aspirin 75mg/d the next day after the operation for total 7 days .patients in the control group were not given anticoagulant after the operation .The patients in both the groups were followed up af_ter surgery for six months ,with the incidence rates of the venous thrombosis compared between the two groups .Results :There were no venous thrombosis in the preoperative DD detection group ,while there were 9 cases of venous thrombosis in the lower extremity in the control group after 6 months ,which showed significant difference between the two groups ( P <0 .05) .Conclusion:Preoperative detection of DD value can guide the administration of postoperative anticoagulant , thus effectively preventing the venous thrombosis after surgery .
