目的:评价关节腔内注射5-氟尿嘧啶(Flurouracil ,5-FU)联合玻璃酸钠(sodium hyaluronate ,SH)对大鼠膝骨关节炎的疗效。方法:将36只Wistar大鼠制成骨关节炎模型后随机分为5-FU加SH组、SH组和对照组工厂组,分别给予5-FU联合S H、S H和生理盐水关节腔内注射,每周1次连续4次,最后一次治疗后1周处死实验动物,观察3组动物软骨和滑膜组织的基质金属蛋白酶(matrix metalloproteinases -1,MMP-1)免疫组化改变;对软骨进行Mankin's评分;比较3组动物血液中白介素-1(interleukin-1,IL -1)的浓度变化。结果:与SH组和对照组比较,5-Fu加SH组软骨病理切片显示软骨细胞凋亡减少,软骨基质结构完整,黏多糖含量多,滑膜细胞减少,滑膜下层的纤维增生减轻;血清中IL -1浓度明显降低( P <0.01);关节软骨中MMP-1表达明显减弱( P <0.01);Mankin's评分有明显改善( P<0.01)。结论:采用关节腔内5-FU加SH注射法治疗大鼠膝骨关节炎,能保护软骨基质,减轻滑膜增生,延缓软骨退行性变的进展。5-FU与SH联合用药可能成为治疗骨性关节炎的一种新的方法。

목적:평개관절강내주사5-불뇨밀정(Flurouracil ,5-FU)연합파리산납(sodium hyaluronate ,SH)대대서슬골관절염적료효。방법:장36지Wistar대서제성골관절염모형후수궤분위5-FU가SH조、SH조화대조조공엄조,분별급여5-FU연합S H、S H화생리염수관절강내주사,매주1차련속4차,최후일차치료후1주처사실험동물,관찰3조동물연골화활막조직적기질금속단백매(matrix metalloproteinases -1,MMP-1)면역조화개변;대연골진행Mankin's평분;비교3조동물혈액중백개소-1(interleukin-1,IL -1)적농도변화。결과:여SH조화대조조비교,5-Fu가SH조연골병리절편현시연골세포조망감소,연골기질결구완정,점다당함량다,활막세포감소,활막하층적섬유증생감경;혈청중IL -1농도명현강저( P <0.01);관절연골중MMP-1표체명현감약( P <0.01);Mankin's평분유명현개선( P<0.01)。결론:채용관절강내5-FU가SH주사법치료대서슬골관절염,능보호연골기질,감경활막증생,연완연골퇴행성변적진전。5-FU여SH연합용약가능성위치료골성관절염적일충신적방법。
Objective :To evaluate the curative effect of intra -articular injection of Flurouracil (5 -FU ) combined with sodium hyaluronate (SH) in the treatment of knee osteoarthritis in rats .Methods :Thirty -six osteoarthritis (OA) model rats were randomly divided into the 5-FU plus SH group ,the SH group and the control group .The three groups were treated with intra -articular injection of 5-FU combined with SH ,SH and physiologic saline ,respectively ,once a week ,for 4 weeks .The model rats were put to death one week after the last injection .The immunochemical changes in matrix metalloproteinases -1 (MMP-1) of cartilage and synovium were observed in the three groups .Mankin's grading system was used for rating of the cartilage .The levels of interleukin -1(IL -1β) in the blood were compared among the three groups .Results :Compared with the SH group and the control group ,the 5-FU with SH group showed reduced apoptosis of chondrocytes and they also kept a more integrated structure of matrix and contained more glycosaminoglycan . The synoviocytes were reduced ;the fibrous hyperplasia in synovial underlayer was inhibited ;The level of IL -1βin the serum dropped( P < 0 .01);the expression of MMP -1 in the cartilage was significantly decreased ( P < 0 .01);the Mankin's score was significantly decreased ( P <0 .01) .Conclusion :Intra -articular injection of 5 -FU combined with SH can relieve OA ,protect the matrix of cartilage ,decrease the synoviocyttes ,inhibit fibrous hyperplasia and delay the cataplasia of the articular cartilage .Thus ,it can be a new way to treat osteoarthritis .
