目的：开发肿瘤患者出院延续性护理信息平台并应用。方法：通过联结医院信息系统（HIS）、门诊诊疗系统等多个模块的专用软件，摘取或移植患者基本信息及有关肿瘤学诊治的相关信息，建立以医院为基础的肿瘤患者出院延续性护理数据库。结果：该信息平台易于操作、实用性强，提供更高质量的肿瘤病例信息资料，确保信息的完整性、正确性。结论：信息技术的开发运用，为肿瘤患者出院延续性护理的开展提供了保障，同时为进行相关研究提供了数据。

Objective:To establish and application post-discharged continuity of care information platform for cancer patients. Methods:By coupling the special software of multiple modules such as the hospital HIS, outpatient clinic system, and extracting the basic information and oncology-related diagnosis and treatment information to establish a hospital-based post-discharged continuity of care information platform for cancer patients. Results:The information platform with practicality was easy to operate, and provided higher quality information material of the cancer cases to ensure information integrity and correctness. Conclusion:The development and application of information technology provide a guarantee for the carrying out of post-discharged continuity of care information platform for cancer patients, as well as providing a basis for cancer research.
