目的:探讨可吸收螺钉张力带固定横行髌骨骨折的生物力学特性及稳定性。方法在12具新鲜膝关节标本上制作横行髌骨骨折模型,分别用可吸收螺钉联合张力带(polydloxanone suture,PDS)(A 组)、AO 张力带(B 组)固定,牵拉股四头肌肌腱,使其从屈曲90°至完全伸直,反复加载后测试髌骨骨折端的位移。结果无论膝关节是处于屈曲位,还是伸直位,除第1次加载,A 组骨折端位移较 B 组大(P <0.05);两组随着循环加载次数的增加,骨折端位移逐渐增加(P <0.05)。A 组均在加载1200次左右固定失败(即位移大于3 mm)。结论可吸收内植物固定横行髌骨骨折无需二次手术,但是其稳定性显著低于克氏针张力带法,需要外固定支具辅助固定。

Objective To explore the biomechanical characters and the stabilization of absorbable screws tension band in the treatment of transverse patella fractures. Methods Transverse patella fractures were created in twelve fresh knees and fixed by one of two techniques:(A Group)absorbable screws with a PDS(Polydloxanone Suture)tension band;(B Group) AO tension band. Testing was performed by pull on the quadriceps tendon between a 90° flexed position and full knee exten_sion,the amount of fracture site displacement in simulated knee extension was measured. Results Except for the first cycle, fracture site displacement in flexion and in extension at A Group was significantly higher than that at B Group(P < 0. 05). With the increase of number of cyclic loading,fracture site displacement at A Group and B Group increased gradually(P < 0. 05). A Group failed over 1 200 cycles loading with a failure criterion of 3 mm fracture site displacement. Conclusion Revi_sion surgery to remove the implants is not required regarding the use of absorbable implants for fixation of patella fractures. But the stabilization of this technique is significantly lower than modified tension band. Auxiliary external fixation is necessary.
