目的:比较妊娠期高血压疾病孕妇与正常孕妇外周血、脐带血和胎盘中转化生长因子(TGF)-β1、血管紧张素(Ang)Ⅱ和血管内皮生长因子(VEGF)的表达差异,为探讨妊娠期高血压疾病的发病机制提供临床依据。方法选择2009年1月至2012年12月在成都市妇女儿童中心医院分娩且合并妊娠期高血压疾病,但无其他妊娠合并症的50例孕妇为研究对象,纳入妊娠期高血压疾病组,选择同期在同一家医院分娩的50例正常孕妇纳入对照组。在孕妇分娩时留取胎盘母、子面胎盘组织,母亲静脉血及新生儿脐静脉血,采用酶联免疫吸附测定(ELISA)法测定 TGF-β1、AngⅡ和 VEGF 的表达水平,并进行统计学分析。两组孕妇年龄、孕龄及体质量比较,差异均无统计学意义(P >0.05)。本研究遵循的程序符合成都市妇女儿童中心医院人体试验委员会制定的伦理学标准,得到该委员会批准,分组征得受试对象知情同意,并与之签署临床研究知情同意书。结果①妊娠期高血压疾病组母亲静脉血、新生儿脐静脉血、胎盘母面及胎盘子面组织中 TGF-β1表达水平均较对照组低,而 AngⅡ表达水平均较对照组高,且差异均有统计学意义(P <0.05);②妊娠期高血压疾病组新生儿脐静脉血、胎盘母面及胎盘子面组织中 VEGF 表达水平均较对照组低,且差异有统计学意义(P <0.05);但两组母亲静脉血 VEGF 表达水平比较,差异无统计学意义(t=0.639,P >0.05)。结论TGF-β1、AngⅡ和 VEGF 表达水平的改变在妊娠期高血压疾病的发生、发展中有重要作用。

목적:비교임신기고혈압질병잉부여정상잉부외주혈、제대혈화태반중전화생장인자(TGF)-β1、혈관긴장소(Ang)Ⅱ화혈관내피생장인자(VEGF)적표체차이,위탐토임신기고혈압질병적발병궤제제공림상의거。방법선택2009년1월지2012년12월재성도시부녀인동중심의원분면차합병임신기고혈압질병,단무기타임신합병증적50례잉부위연구대상,납입임신기고혈압질병조,선택동기재동일가의원분면적50례정상잉부납입대조조。재잉부분면시류취태반모、자면태반조직,모친정맥혈급신생인제정맥혈,채용매련면역흡부측정(ELISA)법측정 TGF-β1、AngⅡ화 VEGF 적표체수평,병진행통계학분석。량조잉부년령、잉령급체질량비교,차이균무통계학의의(P >0.05)。본연구준순적정서부합성도시부녀인동중심의원인체시험위원회제정적윤리학표준,득도해위원회비준,분조정득수시대상지정동의,병여지첨서림상연구지정동의서。결과①임신기고혈압질병조모친정맥혈、신생인제정맥혈、태반모면급태반자면조직중 TGF-β1표체수평균교대조조저,이 AngⅡ표체수평균교대조조고,차차이균유통계학의의(P <0.05);②임신기고혈압질병조신생인제정맥혈、태반모면급태반자면조직중 VEGF 표체수평균교대조조저,차차이유통계학의의(P <0.05);단량조모친정맥혈 VEGF 표체수평비교,차이무통계학의의(t=0.639,P >0.05)。결론TGF-β1、AngⅡ화 VEGF 표체수평적개변재임신기고혈압질병적발생、발전중유중요작용。
Objective To compare the expression level differences of transforming growth factor (TGF)-β1,angiotensin (Ang)Ⅱ and vascular endothelial growth factor (VEGF) in maternal blood, umbilical cord blood and the placenta surface in women with hypertensive disorder complicating pregnancy and normal pregnant women,and provide the clinical evidence for exploring the mechanisms of hypertensive disorder complicating pregnancy.Methods A total of 50 pregnant women with hypertensive disorder complicating pregnancy without other complications who delivered in Chengdu Women′s & Children′s Central Hospital from January 2009 to December 2012 were bringing into study group and 50 normal pregnant women who delivered at the same hospital and same time were bringing into control group. Placenta tissue of maternal and fetal surface,maternal blood and umbilical cord blood during childbirth were obtained.The expression levels of TGF-β1,Ang Ⅱ and VEGF were detected by enzyme linked immunosorbent assay (ELISA) and those expression levels were analyzed statistically.There were no statistically significant differences between two groups in maternal age,gestational age and body weight(P >0.05).This study followed the Ethics Standards of Human Trial Committee of Chengdu Women′s &Children′s Central Hospital,and was approved by the Human Trial Committee.All subjects signed the informed consent.Results ①The expression levels of TGF-β1 in maternal blood,umbilical cord blood,placenta maternal surface and fetal surface in hypertensive disorder complicating pregnancy group were significantly lower than those in control group while the expression levels of Ang Ⅱ were significantly higher,and the differences were statistically significant (P < 0.05 ).② The expression levels of VEGF in umbilical cord blood,placenta maternal surface and fetal surface in hypertensive disorder complicating pregnancy group were significantly lower than those in control group,and the differences were statistically significant (P <0.05);But,there were no statistically significant differences between two groups of VEGF expression level in maternal blood (t =0.639,P >0.05 ).Conclusions The change of expression levels of TGF-β1,Ang Ⅱ and VEGF plays an important role in occurrence and progress of hypertensive disorder complicating pregnancy.
