目的:探讨多层螺旋CT(MSCT)多期增强扫描对肝癌少见强化方式的影像分析及诊断价值。方法回顾性分析21例经手术病理证实或临床确诊的不典型强化的肝癌C T资料。结果21例中,小肝癌9例,结节型10例,巨块型2例。平扫17例呈低密度,4例呈等密度,瘤内密度均匀16例,5例密度不均匀。多期增强检查21例中,动脉期5例呈低密度,等密度4例,高密度12例。门脉期7例呈高密度,10例呈等密度,4例病灶呈低密度。延迟期1例呈高密度,2例呈等密度,18例呈低密度。结论 M SC T的多期增强扫描能反映出肝癌少见的强化方式,对其诊断及鉴别有着重要的意义。

목적:탐토다층라선CT(MSCT)다기증강소묘대간암소견강화방식적영상분석급진단개치。방법회고성분석21례경수술병리증실혹림상학진적불전형강화적간암C T자료。결과21례중,소간암9례,결절형10례,거괴형2례。평소17례정저밀도,4례정등밀도,류내밀도균균16례,5례밀도불균균。다기증강검사21례중,동맥기5례정저밀도,등밀도4례,고밀도12례。문맥기7례정고밀도,10례정등밀도,4례병조정저밀도。연지기1례정고밀도,2례정등밀도,18례정저밀도。결론 M SC T적다기증강소묘능반영출간암소견적강화방식,대기진단급감별유착중요적의의。
Objective To investigate the imaging analysis and diagnostic value of multi‐phase enhancement scanning with multi‐slice spiral CT for the atypically enhanced hepatocellular carcinoma .Methods CT data of 21 cases of the atypically enhanced hepatocellular carcinoma confirmed by operation and pathology were retrospectively analyzed in this work .Re‐sults Of those 21 cases ,9 cases were small hepatocellular carcinoma ,10 cases were nodular type ,and 2 cases were mas‐sive type .On plain scanning ,17 cases were shown low density ,4 cases were shown isodensity ,16 cases were shown hom‐ogeneous density ,and 5 cases were shown uneven density .Of 21 cases underwent multi phase enhanced examination ,in arterial phase ,5 cases were shown low density ,4 cases were shown equal density ,and 12 cases were shown high density ;in the portal venous phase ,7 cases were shown high density ,10 cases were shown isodensity ,and 4 cases were shown low density ;and in delay phase ,1 case was shown high density ,2 cases were shown isodensity ,and 18 cases were shown low density .Conclusion Multi‐phase enhancement scanning with multi‐slice spiral CT can reflect the rare way of strengthe‐ning for hepatoma ,which has important significance to the diagnosis and differential diagnosis .
