目的:探讨二维斑点追踪位移成像在临床诊断扩张型心肌病心肌运动时效性中的价值。方法应用GE Vivid E9超声诊断仪及Echopac高级心血管分析工作站对45例正常人及32例扩张型心肌病患者的左心室心肌17节段长轴位移及位移达峰时间进行定量分析。结果正常组左心室长轴基底段位移最大,心尖段最小,差异有统计学意义;左心室长轴各节段位移达峰时间均位于AVC(主动脉瓣膜关闭)之前。扩心病组左心室长轴各节段位移均减低,心尖段位移小于基底段差异有统计学意义;左心室长轴大部分节段位移达峰时间均位于AVC之后。正常组左心室长轴位移均大于扩心病组并有统计学差异;两组长轴节段位移达峰时间均有统计学差异。结论二维斑点追踪位移成像能为临床评价扩张型心肌病心肌运动时效性提供新的指标参数。

Objective To investigate clinical diagnosis value of dilated cardiomyopathy myocardial motion functions using two‐dimensional speckle tracking displacement imaging .Methods We quantitated longitudinal displacement and time to peak longitudinal displacement of left ventricular 17 segment through 45 normal persons and 32 patients with dilated car‐diomyopathy myocardial using GE Vivid E9 ultrasound unit and Echopac advanced workstation .Results LV longitudinal displacement was significantly decreasing from basal to apical segments in the normal group .The left ventricular segment displacement was greater in the normal group than DCM group with statistically significant difference .LV time to peak longitudinal displacement was located before AVC (aortic vavle colsed)in the normal group ,and after AVC in the DCM group;LV time to peak longitudinal displacement was more significantly delayed in the normal group than DCM group and there was a statistically significant difference between groups .Conclusion Two‐dimensional speckle tracking displacement imaging can provide new parameter for clinical evaluation of dilated cardiomyopathy myocardial motion functions .
