目的：总结肱骨髁间骨折切开复位内固定手术经验。方法对68例肱骨髁间骨折患者行开放复位内固定手术治疗。结果68例患者平均随访时间22个月（14～42个月）。3例老年患者术后鹰嘴截骨处延迟愈合，2例青少年患者经鹰嘴截骨入路术后鹰嘴骨骺损伤，7例出现肘关节异位骨化，5例发生创伤性关节炎，1例出现迟发性尺神经炎。根据Mayo肘关节功能评分平均为85．6分，其中优30例，良25例，可7例，差6例，优良率为80．9％。结论肱骨髁间骨折应根据患者年龄及骨骼质量选择合理的手术方式。成人采用经尺骨鹰嘴截骨入路大多可获得满意的临床疗效，老年严重骨质疏松患者慎用经尺骨鹰嘴截骨入路，骨骺发育未成熟的青少年应减少对骨骺的损伤。

Objective To summary the experience of open reduction and internal fixation for humeral intercondylar fractures.Methods A total of 68 patients humeral intercondylar fractures receivedopen reduction and internal fixation.Results The followup time ranged from 14 to 42 months,with anaverage of 22 months.Three elderly patients with olecranon osteotomy had delayed union,two adolescentpatients with approach of the olecranon osteotomy had olecranon epiphyseal injuries,seven patients had elbow ectopic ossification,five patients had traumatic arthritis,and one patient had delayedonset ulnar neuritis.According to the Mayo Elbow Performance Score,there were excellent in 30,good in 25,fair in 7 andpoor in 6 cases.The average score was 85.6 points and the rate of excellent and good was 80.9%.Conclusion Appropriate surgical approach for humeral intercondylar fractures should be based on the patient's age and bone quality.Adult patients with olecranon osteotomy approach can achieve satisfactory results.Elderly patients with severe osteoporosis should consider the use of olecranon osteotomy cautiously.Damageof the epiphysis should be reduced in adolescents with immature epiphysis.
