目的：分析初诊2型糖尿病患者应用胰岛素泵强化治疗所需的胰岛素剂量以探寻患者初始治疗时可能最适合的预混胰岛素强化方案，为临床治疗方案的选择提供依据。方法回顾分析51例应用胰岛素泵强化治疗的初诊2型糖尿病患者的病历资料，计算患者血糖达标时所需的餐前大剂量与基础量，根据早晚餐前短效和中长效胰岛素比值，推算出可能适合的预混胰岛素强化剂型。结果患者血糖达标时餐前大剂量与基础量的比值为4／5。30例（占总人数的59％）适合应用预混胰岛素50剂型。结论初诊2型糖尿病患者的最佳强化方案应为胰岛素泵或餐时－基础胰岛素；相对于其他剂型的预混胰岛素，预混胰岛素50可能更适合用于初诊2型糖尿病患者的强化治疗。

Objective To analyze the amount of insulin doses needed by newly diagnosed type 2 diabetes pa‐tients who are treated by insulin pump ,to explore the most suitable program to strengthen by premixed insulin. Methods Fifty‐one medical records of patients with newly diagnosed type 2 diabetes mellitus applicated by contin‐uous subcutaneous insulin infusion were retrospectively analyzed. The amount of insulin doses needed by pre‐meal and basis was calculated according to the short‐acting and long‐acting insulin ratio before morning and evening meals when blood glucose reaches the standard ,and got the suitable premixed insulin for intensive therapy. Results When blood glucose reaches the normal standard ,the ratio of insulin doses needed by pre‐meal and basis is 4/5. There are 30 people (59% of the total) suitable for intensive therapy by premixed insulin 50. Conclusion The most suitable intensive treatment is insulin pump or insulin injections four times daily. Compared to other forms of premixed insulin ,premixed insulin 50 is more suitable for patients with newly diagnosed type 2 diabetes.
