目的:探讨窄谱中波紫外线( NB-UVB)联合复方甘草酸苷治疗斑秃的疗效及机制。方法将73例斑秃患者随机分为联合组37例和药物组36例,另选37例健康查体者作为对照组。药物组及联合组均口服复方甘草酸苷50 mg,3次/d,联合组在此基础上行NB-UVB照射治疗,治疗3个月后判定两组疗效。采用流式细胞仪检测三组治疗前后外周血CD3+、CD4+、CD8+及CD4+/CD8+细胞水平变化。结果联合组及药物组有效率分别为91.9%、71.2%,P<0.05。治疗前联合组及药物组CD3+、CD4+及CD4+/CD8+水平均明显低于对照组,P均<0.05;治疗后联合组及药物组CD3+、CD4+及CD4+/CD8+均明显高于治疗前(P均<0.05),CD8+水平无明显变化;联合组CD3+、CD4+及CD4+/CD8+明显高于药物组(P<0.05),CD8+无明显变化。结论斑秃患者免疫功能存在异常,NB-UVB联合复方甘草酸苷治疗斑秃疗效确切;其机制可能为改善患者的免疫功能。

목적:탐토착보중파자외선( NB-UVB)연합복방감초산감치료반독적료효급궤제。방법장73례반독환자수궤분위연합조37례화약물조36례,령선37례건강사체자작위대조조。약물조급연합조균구복복방감초산감50 mg,3차/d,연합조재차기출상행NB-UVB조사치료,치료3개월후판정량조료효。채용류식세포의검측삼조치료전후외주혈CD3+、CD4+、CD8+급CD4+/CD8+세포수평변화。결과연합조급약물조유효솔분별위91.9%、71.2%,P<0.05。치료전연합조급약물조CD3+、CD4+급CD4+/CD8+수평균명현저우대조조,P균<0.05;치료후연합조급약물조CD3+、CD4+급CD4+/CD8+균명현고우치료전(P균<0.05),CD8+수평무명현변화;연합조CD3+、CD4+급CD4+/CD8+명현고우약물조(P<0.05),CD8+무명현변화。결론반독환자면역공능존재이상,NB-UVB연합복방감초산감치료반독료효학절;기궤제가능위개선환자적면역공능。
Objective To investigate the therapeutic effect and mechanism of narrow-band ultraviolet ( NB-UVB ) combined with compound glycyrrhizin in the treatment of alopecia areata .Methods A total of 73 patients with alopecia areata were randomly divided into the drug treatment group (37 patients) and the combined treatment group (36 patients). A total of 37 patients were selected as health control group who did health examination in the same time .The drug treatment group and the combined treatment group were given oral compound glycyrrhizin at 50 mg, 3 times/d.The combined treat-ment group was given NB-UVB radiation therapy besides the oral compound glycyrrhizin .The treatment effectiveness for both groups was evaluated after 3 months.The change of the CD 3+, CD4+and CD4+/CD8+cellular level in peripheral blood was measured by flow cytometry before and after treatment .Results The effective rate of combined treatment group and drug treatment group was 91 .9%and 71 .2%, respectively .The effective rate of combined treatment group was higher than that of drug treatment group ( P<0 .05 ) .Before treatment , compared with the health control group , the cellular level of CD3+, CD4+and CD4+/CD8+of the combine group and the drug treatment group was significantly lower (P<0.05).After treatment , the cellular level of CD 3+, CD4+and CD4+/CD8+of both the combine group and the drug treatment group was sig-nificant higher than before treatment (P<0.05).However, CD8+level was no significant changed in combine group and drug treatment group before and after treatment .The change of cellular level of CD 3+, CD4+and CD4+/CD8+of the combine group was significantly higher than drug treatment group (P<0.05).However, CD8+level was no significant changed in combine group before and after treatment .Conclusions The dysfunction of immune system exists in patients with alopecia areata .NB-UVB combined with glycyrrhizin to treat alopecia areata proved to have a significant curative effect .The mecha-nism of NB-UVB radiation therapy might be able to improve the immunity of alopecia areata patients .
