目的：探讨合并糖尿病的冠心病患者介入术后对比剂肾损伤（CI-AKI）的防治策略。方法回顾性分析2010年12月～2013年11月我院心脏中心明确诊断为“冠心病及糖尿病”，并同意行CAG+PCI患者共100例。根据是否应用丹参川芎嗪分为水化治疗组50例和水化基础上应用丹参川芎嗪治疗组50例。结果研究发现治疗组术后24h与术前比较，血清CysC、Scr、eGFR指标均未见明显差异，差异无统计学意义（P＞0.05）；而对照组术后24h与术前比较，血清CysC明显升高，差异有统计意义（t=5.87，P＜0.05）；治疗组术后24h血清CysC水平明显低于对照组，差异有统计学意义（t=6.01，P＜0.05）。结论血清CysC可较Scr更早期预测CI-AKI，同时对于拟行PCI术的冠心病合并糖尿病患者，应用丹参川芎嗪可在早期改善其肾功能。
목적：탐토합병당뇨병적관심병환자개입술후대비제신손상（CI-AKI）적방치책략。방법회고성분석2010년12월～2013년11월아원심장중심명학진단위“관심병급당뇨병”，병동의행CAG+PCI환자공100례。근거시부응용단삼천궁진분위수화치료조50례화수화기출상응용단삼천궁진치료조50례。결과연구발현치료조술후24h여술전비교，혈청CysC、Scr、eGFR지표균미견명현차이，차이무통계학의의（P＞0.05）；이대조조술후24h여술전비교，혈청CysC명현승고，차이유통계의의（t=5.87，P＜0.05）；치료조술후24h혈청CysC수평명현저우대조조，차이유통계학의의（t=6.01，P＜0.05）。결론혈청CysC가교Scr경조기예측CI-AKI，동시대우의행PCI술적관심병합병당뇨병환자，응용단삼천궁진가재조기개선기신공능。
Objective To investigate the CI-AKI prevention strategies of coronary heart disease (CHD) patients with diabetes mellitus after interventional treatment. Methods The clinical data of 100 cases CHD patients with diabetes mellitus by CAG+PCI were retrospectively analyzed.According to whether the application of Danshen Chuanxiong Injection was divided into hydration treatment group 50 cases and hydration on the basis of application of Danshen Chuanxiong Injection treatment group 50 cases. Results The study found that the treatment group compared with preoperative and postoperative 24 hours,Serum CysC,Scr,eGFR were no significant differences,no tatistical significance(P > 0.05);Compared with the preoperative of the control group after 24 hours,serum CysC increased significantly,the difference had statistical significance (t=5.87,P < 0.05);Compared with the postoperative of the treatment group after 24 hours,serum CysC decreased significantly,the difference had statistical significance(t=6.01, P<0.05). Conclusion Serum CysC can be more early than Scr prediction CI-AKI.Danshen Chuanxiong Injection can improve the early renal function of CHD patients with diabetes mellitus after PCI.
