目的:探讨血清α-L-岩藻糖苷酶(AFU)、亮氨酸氨基肽酶(LAP)、r-谷氨酰转移酶(GGT)联合检测在早期原发性肝癌患者中的诊断价值。方法对85例肝病患者(肝硬化组45例、早期原发性肝癌组40例)及40例健康体检者(健康对照组)的血清 AFU、LAP 及 GGT 含量进行检测。结果早期原发性肝癌组、肝硬化组血清 AFU、GGT 的含量均高于健康对照组(均 P <0.05)。早期原发性肝癌组的 LAP 含量明显高于健康对照组,血清 AFU、LAP、GGT 单项检测及 AFU+LAP+GGT 联合检测阳性率均明显高于肝硬化组及健康体检组(均 P <0.05),其中 AFU+LAP+GGT 联合检测阳性率最高(92%)。肝硬化组与健康对照组 LAP 含量比较差异无统计学意义(P >0.05)。结论AFU+LAP+GGT 联合检测有利于原发性肝癌的早期诊断。

목적:탐토혈청α-L-암조당감매(AFU)、량안산안기태매(LAP)、r-곡안선전이매(GGT)연합검측재조기원발성간암환자중적진단개치。방법대85례간병환자(간경화조45례、조기원발성간암조40례)급40례건강체검자(건강대조조)적혈청 AFU、LAP 급 GGT 함량진행검측。결과조기원발성간암조、간경화조혈청 AFU、GGT 적함량균고우건강대조조(균 P <0.05)。조기원발성간암조적 LAP 함량명현고우건강대조조,혈청 AFU、LAP、GGT 단항검측급 AFU+LAP+GGT 연합검측양성솔균명현고우간경화조급건강체검조(균 P <0.05),기중 AFU+LAP+GGT 연합검측양성솔최고(92%)。간경화조여건강대조조 LAP 함량비교차이무통계학의의(P >0.05)。결론AFU+LAP+GGT 연합검측유리우원발성간암적조기진단。
ABSTRACT:Objective To investigate clinical value of combined detection of serum alpha-L-fu-cose glucosidase(AFU),leucine aminopeptidase(LAP)and r-glutamine transpeptidase(GGT)in the diagnosis of early-stage primary liver cancer.Methods Serum levels of AFU,LAP and GGT were detected in 45 patients with cirrhosis,40 patients with early-stage primary liver cancer and 40 healthy controls.Results Serum levels of AFU and GGT in early-stage primary liver cancer group or cirrhosis group were significantly higher than those in healthy control group(P <0.05). Serum levels of LAP in early-stage primary liver cancer group were significantly higher than those in healthy control group(P <0.05).No significant difference in LAP levels was found between early-stage primary liver cancer group and cirrhosis group(P >0.05).The positive rates of the de-tection of GGT,LAP and AFU alone or their combination in early-stage primary liver cancer group were significantly higher than those in cirrhosis group or healthy control group(P <0.05), in which that of their combination in early-stage primary liver cancer group was highest(92%). Conclusion The combined detection of GGT,LAP and AFU is conducive to the early diagnosis of primary liver cancer.
