目的:对无创性目标潮气量机械通气在COPD合并Ⅱ型呼吸衰竭患者中的应用价值分析。方法临床选取我院呼吸科2013年7月~2014年7月实施治疗的99例COPD合并Ⅱ型呼吸衰竭患者,并将其随机分成3组,其中对照组患者实施常规治疗;ST组患者在常规治疗基础上,加上无创呼吸机ST模式治疗;TVV组患者在常规治疗基础上加上无创呼吸机TVV模式治疗。结果治疗前,3组患者血气情况差异无统计学意义(P>0.05);治疗后3组患者血气均比治疗前有明显改善,差异有统计学意义(P<0.05);治疗后,ST组和TVV组患者和对照组相比,其血气改善效果具有明显优势,差异有统计学意义(P<0.05);治疗后,ST组和TVV组患者血气改善效果对比,差异无统计学意义(P>0.05)。结论无创性目标潮气量机械通气在COPD合并Ⅱ型呼吸衰竭患者临床治疗中的应用,能够取得良好临床效果,值得推广。

Objective To analyze the effects of Noninvasive tidal volume mechanical ventilation in the implementation of COPD with type Ⅱ respiratory failure. Methods 99 cases of COPD with type Ⅱ respiratory failure patients in the implementation of treatment in respiratory department of our hospital from July 2013 - July 2014 were selected by the clinical, and they were randomly divided into three groups, the control group patients were given routine treatment; the ST group patients were given noninvasive ventilator ST model treatment on the basis of routine therapy;while the TVV group patients were given TVV model treatment on the basis of conventional treatment combined with noninvasive ventilator. Results Before treatment, the differences of blood gas of patients in the three groups were not big, they were not statistically significant (P > 0.05); after treatment, the flesh of the three groups of patients was obviously improved than that of before treatment, the difference was significant (P < 0.05); After treatment, the flesh improve effects of patients in ST and TVV group had more obvious advantages than that of the control group, the differences were significant (P < 0.05); After treatment, the difference of effect of the blood gas improved of patients in the ST and TVV group was no significant (P>0.05). Conclusion Noninvasive target tidal volume mechanical ventilation can achieve good clinical application effects in the clinical treatment of patients with COPD with typeⅡrespiratory failure, so is worthy of promoting.
