为观察和分析法医病理学推断人脾破裂损伤时间的方法和特点。本文选取2010年6月至2013年10月120例外伤性脾破裂手术切除标本进行实验研究;按照标本破裂时间,把120例标本分成8组,采用常规染色体观察标本破裂出血区域边缘组织学的变化情况,经免疫组织染色后观察标本破裂出血区域SMA阳性细胞的表达规律。结果显示,损伤时间约6h组的标本有纤维素渗出,7~12h组有大量中性粒细胞浸润,13h~1d组分散少量SMA阳性细胞,1~2d组含有的SMA阳性细胞呈网状分布,2~3d组含有大量SMA阳性细胞,3~7d组和8~14d组出现肉芽组织,跃14d组的标本有瘢痕形成,各组标本SMA阳性细胞的数量差异具有统计学意义(P<0.05)。以脾脏标本破裂出血区域边缘组织学的变化情况,推断人脾破裂损伤时间的法医病理学鉴定方法具有一定的准确性和可靠性。

In order to observe and analyze the methods and characteristics of time estimation of human spleen rupture with forensic pathology, this article selects the surgically resected specimens of 120 cases of trau-matic splenic rupture from June 2010 to October 2013 for experimental research; according to the rupture time, the specimens were divided into 8 groups, histologic changes of rupture bleeding edge were observed by the con-ventional chromosome, and SMA positive cells expression patterns of rupture bleeding area were observed after immunohistochemical staining. The results show: the group with injury time < 6 h has fibrinous exudates, the group with injury time of 7-12 h has a large number of neutrophil infiltration, the group with injury time of 13h-1d is scattered with small amounts of SMA positive cells, the group with injury time of 1-2d has net-like distri-bution of SMA positive cells, the group with injury time of 2-3d contains a lot of SMA positive cells, the group with injury time of 3-7d and the group with injury time 8-14 d have dgranulation tissue, the group with injury time>14d has scarring, and the number difference of each group's SMA positive cells is of statistical significance(P<0.05). Therefore, forensic pathology appraisal method of predicting injury time of human spleen rupture with histologic changes of rupture bleeding edge has certain accuracy and reliability.
