目的：探讨氧与药物联合应用治疗颅脑外伤后头痛的效果。方法对2010年4月至2012年3月共105例颅脑损伤后头痛患者进行氧与药物联合应用的对照研究。经患者知情同意，医院伦理委员会批准，将105例患者分为3组：高压氧组（42例），舱外高流量氧组（36例）和对照组（27例）。以疼痛视觉模拟评分（VAS）作为头痛治疗效果评价方法。3组患者均进行常规药物治疗，即应用改善微循环、营养神经、镇痛等药物，高压氧组加用高压氧治疗，舱外高流量组加用舱外高流量氧治疗（常压氧）。结果治疗2个疗程后高压氧组、舱外高流量氧组、对照组的VAS评分别为（1．24±1．63）、（3．21±2．12）、（4．13±1．42）分；头痛次数为（1．67±0．62）、（3．87±1．29）、（4．28±0．75）次／周、头痛持续时间分别为（1．48±0．87）、（2．52±0．92）、（4．37±0．96）h。各组比较差异均有统计学意义（P＜0．05）。3组患者中高压氧组治愈率最高，舱外高流量氧组效果次之，前2组疗效均优于对照组，各组之间比较差异有统计学意义（P＜0．05）。结论对颅脑创伤后头痛患者，联合应用氧与药物治疗是既有效又安全的治疗方案。
목적：탐토양여약물연합응용치료로뇌외상후두통적효과。방법대2010년4월지2012년3월공105례로뇌손상후두통환자진행양여약물연합응용적대조연구。경환자지정동의，의원윤리위원회비준，장105례환자분위3조：고압양조（42례），창외고류량양조（36례）화대조조（27례）。이동통시각모의평분（VAS）작위두통치료효과평개방법。3조환자균진행상규약물치료，즉응용개선미순배、영양신경、진통등약물，고압양조가용고압양치료，창외고류량조가용창외고류량양치료（상압양）。결과치료2개료정후고압양조、창외고류량양조、대조조적VAS평분별위（1．24±1．63）、（3．21±2．12）、（4．13±1．42）분；두통차수위（1．67±0．62）、（3．87±1．29）、（4．28±0．75）차／주、두통지속시간분별위（1．48±0．87）、（2．52±0．92）、（4．37±0．96）h。각조비교차이균유통계학의의（P＜0．05）。3조환자중고압양조치유솔최고，창외고류량양조효과차지，전2조료효균우우대조조，각조지간비교차이유통계학의의（P＜0．05）。결론대로뇌창상후두통환자，연합응용양여약물치료시기유효우안전적치료방안。
Objective To explore the effects of oxygen coupled with drug therapy in the treatment of headache following trau -matic brain injury.Methods A comparative study was made on 105 cases of headache induced by traumatic brain injury treated with hyperbaric oxygen ( HBO) coupled with drug therapy from April 2010 to March 2012.The patients were divided into 3 groups:the HBO group (42 cases), the outside chamber high-flow oxygen group (36 cases) and the control group (27 cases).The pain visual analog scores ( VAS) were used as a pain assessment method for the treatment efficacy of headache .The patients of 3 groups were all given routine drug treatment , for improvement of microcirculation , nutrition of neural cells and analgesia .The patients in the HBO and the outside chamber high-flow oxygen groups were respectively supplemented with HBO therapy and high-flow oxygen therapy .Results Af-ter two treatment courses , VAS scores for the HBO group , the outside chamber high-flow oxygen group and the control group were re-spectively (1.24 ±1.63),(3.21 ±2.12)and(4.13 ±1.42).The number of headache for the 3 groups were respectively(1.67 ± 0.62)times/week,(3.87 ±1.29)times/week and(4.28 ±0.75)times/week.And the durations of headache were respectively (1.48 ± 0.87)h,(2.52 ±0.92)h and(4.37 ±0.96) h.Statistical significance could be noted , when comparisons were made between the 3 groups (P<0.05).Results indicated that cure rate of the HBO group was the highest , the outside chamber high-flow oxygen group came the second , which were all superior to that of the control group , and statistical difference could be seen , when comparisons were made between the groups(P<0.05).Conclusion For patients with headache following traumatic brain injury , the combined treatment with oxygen and drugs was a safe and effective treatment method .
