目的:观察川崎病( KD)患儿血清血管生长因子( VEGF)及内皮抑素( ES)的变化,探讨其在KD发病机制中的作用及与冠状动脉扩张的相关性。方法以30例健康儿童为对照组,将32例初发KD患儿根据心脏彩超结果分为冠状动脉病变组( CAL组)12例,无冠状动脉病变组( NCAL组)20例,采用酶联免疫法检测两组患儿血清VEGF及ES的水平,运用超声心动图检测冠状动脉扩张程度,分析血清VEGF、ES水平及VEGF/ES变化及其与冠状动脉扩张程度的相关性。结果两组患儿急性期和恢复期血清VEGF、ES水平及VEGF/ES均高于对照组,差异均有统计学意义( P﹤0.05);CAL组急性期和恢复期血清VEGF及ES水平明均显高于NCAL组,差异均有统计学意义( P ﹤0.05),且患儿血清VEGF、ES水平及VEGF/ES与冠状动脉扩张程度均呈显著正相关( P ﹤0.01)。结论 VEGF及ES可能参与KD患儿急性期血管炎的发病机制,并且对于评估KD冠状动脉损伤具有良好的临床价值。

목적:관찰천기병( KD)환인혈청혈관생장인자( VEGF)급내피억소( ES)적변화,탐토기재KD발병궤제중적작용급여관상동맥확장적상관성。방법이30례건강인동위대조조,장32례초발KD환인근거심장채초결과분위관상동맥병변조( CAL조)12례,무관상동맥병변조( NCAL조)20례,채용매련면역법검측량조환인혈청VEGF급ES적수평,운용초성심동도검측관상동맥확장정도,분석혈청VEGF、ES수평급VEGF/ES변화급기여관상동맥확장정도적상관성。결과량조환인급성기화회복기혈청VEGF、ES수평급VEGF/ES균고우대조조,차이균유통계학의의( P﹤0.05);CAL조급성기화회복기혈청VEGF급ES수평명균현고우NCAL조,차이균유통계학의의( P ﹤0.05),차환인혈청VEGF、ES수평급VEGF/ES여관상동맥확장정도균정현저정상관( P ﹤0.01)。결론 VEGF급ES가능삼여KD환인급성기혈관염적발병궤제,병차대우평고KD관상동맥손상구유량호적림상개치。
Objective To observe the changes of serum vascular endothelial growth factor( VEGF)and endostatin( ES)of patients with kawasaki disease and to investigate the correlation between the role of change of serum VEGF and ES in KD and coronary artery dilation. Methods 30 cases of healthy children were selected as control group,32 cases of children patients with KD were divided into coronary artery lesion group ( CAL group,with 12 cases)and non coronary artery lesion group( non CAL group,with 20 cases). Enzyme-linked immunoassay detection was used to detect the level of VEGF and ES. The echocardiography was used to detect the degree of coronary artery dilation,and correlation between changes of serum VEGF and ES,VEGF/ES and degree of coronary artery dilation. Results The level of serum VEGF,ES and VEGF/ES in a_cute phase and convalescence phase of two patients group was higher than that of control group. There was statistically significant difference( P ﹤0. 05). Level of serum VEGF and ES of CAL group in acute phase and convalescence phase was obviously higher than that of NCAL group,with statistically significant difference( P ﹤0. 05). Patients′level of serum VEGF,ES and VEGF/ES was significantly positively correlated with coro_nary artery dilation( P ﹤0. 01). Conclusion VEGF and ES are possibly associated with the disease of acute vasculitis of patients with KD and have good clinic value to evaluate coronary artery injury of patients with KD.
