目的:观察不同部位注射造影剂对多层螺旋CT冠状动脉成像检查图像效果的影响。方法选取115例拟行冠脉造影的患者,依造影剂注射部位,随机分为:左肘正中静脉组(20例)、右肘正中静脉组(49例)、右前臂贵要静脉组(23例)和右前臂头静脉桡侧组(23例)。观察各组冠状动脉成像质量。结果经右侧肘正中静脉团注成功后冠状动脉显影效果高于左侧,差异有统计学意义( P <0.01)。右侧肘正中静脉和右侧前臂贵要静脉团注成功后冠状动脉显影效果好,其次是右侧前臂头静脉桡侧。三组注射部位之间图像质量差异有显著统计学意义( P <0.01)。结论为提高冠状动脉成像质量,在进行增强CT检查时右侧肘正中静脉为首选注射部位。其次是右侧前臂贵要静脉,再次可以考虑右侧前臂头静脉桡侧。

목적:관찰불동부위주사조영제대다층라선CT관상동맥성상검사도상효과적영향。방법선취115례의행관맥조영적환자,의조영제주사부위,수궤분위:좌주정중정맥조(20례)、우주정중정맥조(49례)、우전비귀요정맥조(23례)화우전비두정맥뇨측조(23례)。관찰각조관상동맥성상질량。결과경우측주정중정맥단주성공후관상동맥현영효과고우좌측,차이유통계학의의( P <0.01)。우측주정중정맥화우측전비귀요정맥단주성공후관상동맥현영효과호,기차시우측전비두정맥뇨측。삼조주사부위지간도상질량차이유현저통계학의의( P <0.01)。결론위제고관상동맥성상질량,재진행증강CT검사시우측주정중정맥위수선주사부위。기차시우측전비귀요정맥,재차가이고필우측전비두정맥뇨측。
Objective To discuss the influence of different site of intravenous injection of contrast media on imaging effect of multi-slice computed tomography( MSCT)coronary angiography. Methods One hundred and fifteen patients were randomly divided into four groups( left median cubital vein group with 20 patients,right median cubital vein group with 49 patients,right basilic vein group with 23 patients,and right cephalic vein group with 23 patients)according to different site of intravenous bolus injection before undergoing MSCT coronary angiography. Then the acquired images were observed and analyzed for quality by radiologists. Results There was significant difference between the left and right median cubital vein groups( P <0. 01). The image quality was higher in the right median cubital vein group. There was significant difference a-mong the three groups of different injection sites in right arm( P <0. 01). The image quality was higher in the right median cubital vein group and the right basilic vein group than in the right radial cephalic vein group. Conclusion The right median cubital intravenous injection is suggested to be the first choice to obtain higher image quality of MSCT. The second choice is right basilic vein,while the right radial cephalic vein will be the last one.
