目的:探讨双源CT冠状动脉钙化评分对诊断冠状动脉狭窄的价值。方法:回顾性分析在笔者所在医院2012年6月-2013年12月行双源CT冠状动脉造影检查的84例患者,在DSCT图像中评估钙化积分,以及传统冠状动脉造影Gensini评分,利用ROC工作特征曲线,以传统冠状动脉造影检查结果为参考标准,评估双源CT冠状动脉钙化积分预测冠状动脉管腔显著狭窄的诊断能力。结果:钙化积分与Gensini评分之间存在正相关关系(r=0.718,P<0.001),ROC曲线分析显示,钙化积分最佳阈值为321时,检测冠状动脉狭窄≥50%的敏感度、特异度及曲线下面积分别为85%、77%、0.868。结论:钙化积分对预测冠状动脉中度以上狭窄有很高的价值,可为临床诊断冠心病提供一项可靠的无创检查手段。
目的:探討雙源CT冠狀動脈鈣化評分對診斷冠狀動脈狹窄的價值。方法:迴顧性分析在筆者所在醫院2012年6月-2013年12月行雙源CT冠狀動脈造影檢查的84例患者,在DSCT圖像中評估鈣化積分,以及傳統冠狀動脈造影Gensini評分,利用ROC工作特徵麯線,以傳統冠狀動脈造影檢查結果為參攷標準,評估雙源CT冠狀動脈鈣化積分預測冠狀動脈管腔顯著狹窄的診斷能力。結果:鈣化積分與Gensini評分之間存在正相關關繫(r=0.718,P<0.001),ROC麯線分析顯示,鈣化積分最佳閾值為321時,檢測冠狀動脈狹窄≥50%的敏感度、特異度及麯線下麵積分彆為85%、77%、0.868。結論:鈣化積分對預測冠狀動脈中度以上狹窄有很高的價值,可為臨床診斷冠心病提供一項可靠的無創檢查手段。
목적:탐토쌍원CT관상동맥개화평분대진단관상동맥협착적개치。방법:회고성분석재필자소재의원2012년6월-2013년12월행쌍원CT관상동맥조영검사적84례환자,재DSCT도상중평고개화적분,이급전통관상동맥조영Gensini평분,이용ROC공작특정곡선,이전통관상동맥조영검사결과위삼고표준,평고쌍원CT관상동맥개화적분예측관상동맥관강현저협착적진단능력。결과:개화적분여Gensini평분지간존재정상관관계(r=0.718,P<0.001),ROC곡선분석현시,개화적분최가역치위321시,검측관상동맥협착≥50%적민감도、특이도급곡선하면적분별위85%、77%、0.868。결론:개화적분대예측관상동맥중도이상협착유흔고적개치,가위림상진단관심병제공일항가고적무창검사수단。
Objective:To investigate the value of coronary calcium score assessed by dual-source computed tomography coronary angiography in the diagnosis of coronary artery stenosis.Method:The DSCT scanned data of 84 suspected coronary heart disease patients in our hospital from June 2012 to December 2013 were retrospectively analyzed,including CACS of coronary artery CT angiography by dual-source CT and CAG,Gensini score was used to evaluate the degree stenosis of coronary artery.The correlation between the two methods was evaluated using Spearman correlation analysis.With CAG as the reference standard, the capability to predict obvious coronary stenosis was compared using ROC curve.Result:There was a positive association between calcium score and Gensini score(r=0.718,P<0.001).Calcium scoring had an acceptable value for diagnose CA stenosis≥50% with optimal cutoff point of 321,yielding a sensitivity, specificity and the area under the AUC were 85%,77% and 0.868,respectively.Conclusion:Calcium score for predicting coronary artery above moderate stenosis has a high value,for the clinical diagnosis of coronary heart disease and provide a reliable noninvasive method.
