目的：探讨不同疗程无抽搐电休克治疗（MECT）对精神分裂症患者认知功能的影响。方法将符合条件的精神分裂症患者随机分为两组，N1组给予6次 MECT 治疗（n ＝46），N2组给予12次 MECT 治疗（n ＝44）。在治疗前后分别进行威斯康星卡片分类试验（WCST-M）、Stroop 测验、连线测试（TMT），并行阳性与阴性症状量表（PANSS）评定。结果①与基线时相比，MECT 治疗后两组 PANSS 总分及各分量表评分均明显下降（P ＜0．05），且 N2组比 N1组的下降更明显（P ＜0．05）。②与基线时相比，两组 MECT 治疗后 WCST-M 的总测验数、正确反映数、持续错误数、非持续错误数及分类完成数差异有统计学意义（P ＜0．05）。而且 N2组的 WCST-M 总测验数、正确反映数、持续错误数及分类完成数均明显优于 N1组（P ＜0．05）。③与基线时相比，MECT 治疗后两组 Stroop CW 总反映时间、CW 错误数、C 总时间、C 总错误数测验及 TMT-A 和 B 差异有统计学意义（P ＜0．05）。MECT 治疗后相比，N2组 CW 总反映时间、C 总错误数测验及 TMT-A 和 B 与 N1组差异有统计学意义（P ＜0．05）。结论 MECT治疗后，精神分裂症患者的认知功能有一定程度的改善。
목적：탐토불동료정무추휵전휴극치료（MECT）대정신분렬증환자인지공능적영향。방법장부합조건적정신분렬증환자수궤분위량조，N1조급여6차 MECT 치료（n ＝46），N2조급여12차 MECT 치료（n ＝44）。재치료전후분별진행위사강성잡편분류시험（WCST-M）、Stroop 측험、련선측시（TMT），병행양성여음성증상량표（PANSS）평정。결과①여기선시상비，MECT 치료후량조 PANSS 총분급각분량표평분균명현하강（P ＜0．05），차 N2조비 N1조적하강경명현（P ＜0．05）。②여기선시상비，량조 MECT 치료후 WCST-M 적총측험수、정학반영수、지속착오수、비지속착오수급분류완성수차이유통계학의의（P ＜0．05）。이차 N2조적 WCST-M 총측험수、정학반영수、지속착오수급분류완성수균명현우우 N1조（P ＜0．05）。③여기선시상비，MECT 치료후량조 Stroop CW 총반영시간、CW 착오수、C 총시간、C 총착오수측험급 TMT-A 화 B 차이유통계학의의（P ＜0．05）。MECT 치료후상비，N2조 CW 총반영시간、C 총착오수측험급 TMT-A 화 B 여 N1조차이유통계학의의（P ＜0．05）。결론 MECT치료후，정신분렬증환자적인지공능유일정정도적개선。
ABSTRACT:Objective To investigate the effect of different periodical modified electroconvulsive therapy (MECT)on the cognitive functions of patients with schizophrenia.Methods Ninty-eight patients with schizophrenia were randomly divided into two groups:group N1(n =46)and group N2 (n =44).Patients in group N1 received 6 periods of MECT and patients in group N2 12 periods of treatment.Before and after the treatment,Wisconsin Card Sorting Test(WCST),Stroop test,trail making test A and B and positive and negative syndrome scale(PANSS)were performed on each patient to assess the cognitive functions.Results Compared with the baseline,the total scores of PANSS and other scores were significantly reduced by MECT treatment in both groups,especially in group N2 (P <0.05);the results of WCST,TMT and Stroop test improved markedly after MECT treatment in both groups, especially in group N2 (P <0.05).Compared with group N1,the WCST,TMT and Stroop test in N2 group were all better than those of group N1 (P <0.05).Conclusion MECT treatment significantly improves the cognitive functions of patients with schizophrenia.
