目的:探讨IL-6、IL-10、PLA2在重度烧伤并发脓毒症中的变化及其对预后的影响。方法选取2012年2月至2014年6月期间郑州市第一人民医院收治的重度烧伤患者100例,其中30例未合并脓毒血症(A组),70例合并脓毒血症,52例未并发MODS为预后良好组(B组),18例并发MODS为预后不良组(C组)。检测受试者不同时间点血清IL-6、IL-10、PLA2含量。结果3组IL-6水平均在烧伤后7 d出现明显变化(P<0.05),烧伤后7 d,B、C组IL-6水平明显高于A组(P<0.05),且C组IL-6水平高于B组(P<0.05)。A组IL-6水平在烧伤后7 d开始下降(P<0.05),B组IL-6水平在烧伤后14 d开始明显下降(P<0.05),C组IL-6水平则一直处于上升趋势(P<0.05)。3组IL-10水平均在烧伤后7 d出现明显变化(P<0.05),烧伤后7 d,B、C组IL-10水平明显高于A组(P<0.05),且C组IL-10水平高于B组(P<0.05)。A组IL-10水平在烧伤后7 d开始下降(P<0.05),B组IL-10水平在烧伤后14 d开始明显下降(P<0.05),C组IL-10水平则一直处于上升趋势(P<0.05)。3组IL-10水平均在烧伤后3 d出现明显变化(P<0.05),烧伤7 d后变化幅度加大(P<0.05),烧伤后7 d,B、C组PLA2水平明显高于A组(P<0.05),且C组PLA2水平高于B组(P<0.05)。A组PLA2水平在烧伤后7 d开始下降(P<0.05),B、C组PLA2水平在烧伤后14 d开始明显下降(P<0.05),C组IL-10水平则一直处于上升趋势(P<0.05)。结论 IL-6、IL-10、PLA2等指标与重度烧伤并发脓毒症的发生及患者预后密切相关,因此宜对重度烧伤患者相关指标进行动态监测,以及时判断患者病情及预后。

ObjectiveTo study influence of the changes of serum IL-6, IL-10, PLA2 on severe burn complicated with sepsis and prognosis.MethodsFrom February 2012 to June 2014,100 cases of severe burn patients in Zhengzhou First People's Hospital were selected as the research object. 100 cases of severe burn patients, 70 cases were complicated with sepsis, 30 cases not associated with sepsis (A group), 70 cases of sepsis patients, 52 cases were not complicated with MODS, for the good prognosis group (B group), 18 patients with MODS, for the poor prognosis group (group C). Serum IL-6, IL-10, PLA2 content of the subjects in different time point were detected.ResultsThree groups' IL-6 average after burn 7d had obvious changed(P<0.05), 7d after burn, IL-6 level of B group and C group was significantly higher than group A (P<0.05), and C group' IL-6 was higher than group B (P<0.05). The IL-6 level of A group in the postburn 7d declined (P<0.05), the IL-6 level of B group began decreasing 14d in postburn (P<0.05), and the IL-6 level of C group has been in a rising trend (P<0.05). The level of IL-10 in three groups after burn 7d obviously changed (P<0.05), 7d after burn, IL-10 level of B group and C group was significantly higher than group A (P<0.05), and IL-10 of C group was higher than group B (P<0.05). The IL-10 level of A group in the postburn 7d declined (P<0.05), the IL-10 level of B group began decrease 14d in post burn (P<0.05), the IL-10 level of C group has been in a rising trend (P<0.05). The level of IL-10 in three groups after burn 3d obviously changed (P<0.05), 7d after burn change increased (P<0.05), 7d after burn, PLA2 level of B group and C group was significantly higher than group A (P<0.05), and C group' PLA2 was higher than group B (P<0.05). The PLA2 level of A group in the postburn 7d declined (P<0.05), B group, C group, PLA2 level began to decline significantly after burn injury in 14d (P<0.05), IL-10 level of C group has been in a rising trend (P<0.05).ConclusionIL-6, IL-10, PLA2 are closely related to the occurrence and prognosis of patients with severe burn sepsis.It is suitable for patients with severe burn to dynamicly monitor the related indicators, and to judge the condition and prognosis of patients.
