目的：探讨人附睾蛋白4（HE4）在子宫内膜癌中的表达及其诊断价值。方法选择2012年10月至2013年12月福建省肿瘤医院妇科收治的经过病理证实的子宫内膜癌患者88例，另选同期于该院体检中心体检合格的健康妇女42名作为正常对照组，采用直接化学发光法检测两组的血清HE4水平，分析比较其差异以及该指标作为诊断子宫内膜癌的临床应用价值。结果子宫内膜癌组与健康对照组血清HE4水平分别为（60.4±12.6） pmol/L、（40.4±6.5） pmol/L，两组差异有统计学意义（P<0.05）。根据贝叶斯定理绘制ROC曲线，曲线下面积AUC=0.94，根据ROC曲线确定最佳筛查阳性界值（cut-off值）为HE4=56.05 pmol/L，该cut-off值下诊断子宫内膜癌的敏感性为88.6%，特异性为69.4%。结论子宫内膜癌患者血清HE4水平显著高于正常人，且可作为诊断子宫内膜癌的生物学标志物。
목적：탐토인부고단백4（HE4）재자궁내막암중적표체급기진단개치。방법선택2012년10월지2013년12월복건성종류의원부과수치적경과병리증실적자궁내막암환자88례，령선동기우해원체검중심체검합격적건강부녀42명작위정상대조조，채용직접화학발광법검측량조적혈청HE4수평，분석비교기차이이급해지표작위진단자궁내막암적림상응용개치。결과자궁내막암조여건강대조조혈청HE4수평분별위（60.4±12.6） pmol/L、（40.4±6.5） pmol/L，량조차이유통계학의의（P<0.05）。근거패협사정리회제ROC곡선，곡선하면적AUC=0.94，근거ROC곡선학정최가사사양성계치（cut-off치）위HE4=56.05 pmol/L，해cut-off치하진단자궁내막암적민감성위88.6%，특이성위69.4%。결론자궁내막암환자혈청HE4수평현저고우정상인，차가작위진단자궁내막암적생물학표지물。
Objective To explore the diagnosis and treatment values of human epididymis protein 4 (HE4) levels in endometrial carcinoma.Methods From October 2012 to December 2013,the method of direct chemiluminescence assay was applied to detect serum HE4 levels in 88 patients of endometrial cancer (endometrial carcinoma group) and 42 healthy women (control group) in the Fujian Provincial Tumor Hospital, as a tumor marker in the diagnosis and treatment of endometrial carcinoma, the diagnosis and treatment values of HE4 levels were analyzed.Results Serum HE4 levels in endometrial carcinoma group and healthy controls were (60.4±12.6) pmol/L and (40.4±6.5) pmol/L, endometrial cancer group was significantly higher than that of normal control group and the difference was statistically significant (P<0.05); ROC curve drawing according to bayes' theorem, the area under the curve AUC = 0.94, the cut-off the best diagnosis value of HE4 = 56.05 pmol/L, the cut - off value under the sensitivity of the diagnosis of endometrial carcinoma was 88.6%, specificity of 69.4%.Conclusion Endometrial cancer patients serum HE4 were significantly higher than that of normal people, and can be used as biological markers of diagnosis of endometrial carcinoma.
