目的:研究多层螺旋CT(MSCT)对肺炎型肺癌的影像及病理特征,提高诊治水平。方法回顾性分析经病理证实的35例肺炎型肺癌的CT及临床资料,观察病灶分布及影像学表现,再由MSCT后处理软件进行分析。结果影像学特点为:磨玻璃密度影;片结影;肺叶、肺段实变阴影;空泡及蜂窝状阴影;上述影像混合型阴影。病理类型有:肺腺癌、肺鳞癌、细支气管肺泡癌、肺小细胞癌、肺低分化癌。结论肺炎型肺癌CT表现多样性,但其动态影像学亦有一定特征性,一定程度上反映其病理变化过程。通过MSCT各种成像技术的不同重点显示病变,可提供较明确的诊断及鉴别诊断依据。

목적:연구다층라선CT(MSCT)대폐염형폐암적영상급병리특정,제고진치수평。방법회고성분석경병리증실적35례폐염형폐암적CT급림상자료,관찰병조분포급영상학표현,재유MSCT후처리연건진행분석。결과영상학특점위:마파리밀도영;편결영;폐협、폐단실변음영;공포급봉와상음영;상술영상혼합형음영。병리류형유:폐선암、폐린암、세지기관폐포암、폐소세포암、폐저분화암。결론폐염형폐암CT표현다양성,단기동태영상학역유일정특정성,일정정도상반영기병리변화과정。통과MSCT각충성상기술적불동중점현시병변,가제공교명학적진단급감별진단의거。
Objective To study the imaging performance of MSCT for the pneumonic type lung cancer and to improve the diagnosis. Methods This paper retrospectively studied 35 patients with pneumonic type lung cancer confirmed pathologically. All subjects underwent MSCT scanning. The imaging results were evaluated together with clinical history to assess proliferation scope of cancer lesion. The MSCT post-processing software was used for analy-sis. Results Imaging features included ground glass opacity, patchy consolidation, consolidation of lung lobe and lung segment, granular and honeycombing appearance, and combination of multiple features. Pathological types in-cluded lung adencarcinoma, lung squamous cell carcinoma, bronchiolo-alveolar carcinoma, small cell lung carcinoma and poorly differentiated lung carcinoma. Conclusion Although pneumonic type lung cancer has variant imaging ap-pearance in CT, there are also certain imaging features showing the pathological development of this disease. The MSCT imaging can provide imaging evidences for the diagnosis and evaluation.
