目的:采用Johnston分析法评价下颌前伸矫治器(SUS2)矫治安氏II类错牙合的效果。方法:应用SUS2配合MBT矫治器矫治安氏II类错牙合、下颌后缩病例22例(男9例,女13例,年龄13~17岁),X线头影测量比较治疗前后的咬合关系和软组织面型,上下颌骨与牙齿的矢状位置变化。结果:矫治后覆牙合、覆盖和磨牙关系均达到正常,面部软组织凸度和上下唇距审美平面有显著改善,SNB明显增加,ANB显著减小(P<0.05〉);上下颌骨和牙齿在矢状方向上移动明显,上颌骨前移0.99 mm,下颌骨前移4.30 mm,上下颌骨相对位置改变了3.32 mm(P<0.001);上磨牙及上切牙相对上颌骨分别后移0.12 mm、0.60 mm,下磨牙及下切牙相对下颌骨分别前移1.02 mm、1.41 mm(P <0.001),磨牙关系(U6/L 6)改变了4.46 mm(P<0.001),前牙覆盖(U1/L 1)改变了5.33 mm(P<0.001)。结论:SUS2配合MBT矫治器使下颌相对上颌发生明显前移,从而改善咬合关系和软硬组织面型;矫治过程中骨骼变化量明显大于牙齿变化量,SUS2矫治效果主要由骨骼变化引起。

Objective:To assess the effects of using SUS2 on the Class II malocclusion with Johnston and common cephalometric analysis. Method:Twenty-two patients with Class II malocclusion were selected and treated with SUS2 and MBT appliance. Cephalometric analysis were used to evaluate the effects before and after the treatment. Result:Overbite, overjet and relationship of molars got correction after the treatment,soft tissue convexity,Ul-E and Ll-E decreased,profile for those retruded mandible improved significantly (P<0.05).There were significant corrections in sagittal direction,SNB in-creased and ANB decreased significantly. The correction was due more to the mandible than the maxilla (4.30 mm and 0.99 mm,repectively),resulting in an ABCH of 3.32 mm (P<0.001). The upper molars and incisors move backward by 0.12 mm and 0.60 mm. The lower molars and incisor move forward by 1.02 mm and 1.41 mm.These skeletal and dental changes result in a 6/6 of 4.46mm and a 1/1 of 5.33 mm. Conclusion:SUS2 and MBT appliance can promote significant development of mandible in relation to maxilla,improving occlusion relationship and profile of both soft and hard tissues. The correction was due to the combination of skeletal and dental changes,while the majority of correction is skeletal.
