目的::通过检测不同阶段糖尿病肾病患者血清超敏C反应蛋白(hsCRP)和白细胞介素-6(IL-6)水平,探讨微炎症与糖尿病肾病发生和发展的关系。方法:测定健康人(NC组)21例、2型糖尿病不合并糖尿病肾病患者(DM组)21例和糖尿病肾病患者(DN组)65例的血清hsCRP、IL-6水平并比较;将DN组患者按24 h尿蛋白定量1.0 g、3.5 g为标准分为3组:小量蛋白尿组(<1.0 g/d)30例;中量蛋白尿组(1.0~3.5 g/d)22例;大量蛋白尿组(>3.5 g/d)13例及GFR水平分组,并比较各组间血清hsCRP和IL-6水平。结果:血清hsCRP和IL-6水平,DM组、DN组患者明显高于NC组,DN组患者明显高于DM组。不同蛋白尿组间,血清hsCRP和IL-6水平,中量和大量蛋白尿组明显高于NC组、DM组和小量蛋白尿组。不同GFR组间,血清hsCRP水平无明显差别;血清IL-6水平, GFR 15-29 ml/min DN组、GFR<15 ml/min DN组明显高于其余各组,但组间无明显差别。结论:糖尿病肾病患者存在微炎症状态,炎症反应参与了DN的发生和发展。

목적::통과검측불동계단당뇨병신병환자혈청초민C반응단백(hsCRP)화백세포개소-6(IL-6)수평,탐토미염증여당뇨병신병발생화발전적관계。방법:측정건강인(NC조)21례、2형당뇨병불합병당뇨병신병환자(DM조)21례화당뇨병신병환자(DN조)65례적혈청hsCRP、IL-6수평병비교;장DN조환자안24 h뇨단백정량1.0 g、3.5 g위표준분위3조:소량단백뇨조(<1.0 g/d)30례;중량단백뇨조(1.0~3.5 g/d)22례;대량단백뇨조(>3.5 g/d)13례급GFR수평분조,병비교각조간혈청hsCRP화IL-6수평。결과:혈청hsCRP화IL-6수평,DM조、DN조환자명현고우NC조,DN조환자명현고우DM조。불동단백뇨조간,혈청hsCRP화IL-6수평,중량화대량단백뇨조명현고우NC조、DM조화소량단백뇨조。불동GFR조간,혈청hsCRP수평무명현차별;혈청IL-6수평, GFR 15-29 ml/min DN조、GFR<15 ml/min DN조명현고우기여각조,단조간무명현차별。결론:당뇨병신병환자존재미염증상태,염증반응삼여료DN적발생화발전。
Objective:To explore the relationship of the progression of diabetic nephropathy and micro-inflammatory state by detecting serum high-sensitivity c-reactive protein and interleukin-6 levels of diabetic nephropathy patients in different stages. Meth-ods:The serum high-sensitivity C-reactive protein (hsCRP) and interleukin-6 (IL-6) levels of 21 healthy individuals (NC group), 21 cases with type 2 diabetics not combined with diabetic nephropathy ( DM group) and 65 patients with diabetic nephropathy ( DN group) were detected and compared. According to 24-hour urine protein (1. 0 g and 3. 5 g), DN group was divided into three sub-groups:small amount of proteinuria group (<1. 0 g/d), medium (1. 0~3. 5 g/d) and massive proteinuria (>3. 5 g/d) groups, and their serum hsCRP and IL-6 levels were compared. Results:The serum hsCRP and IL-6 levels of DM group and DN group were sig-nificantly higher than those of NC group, and those of DN group were significantly higher than those of DM group. The serum hsCRP and IL-6 levels of medium and massive proteinuria groups were significantly higher than those of NC, DM, and small amount of pro-teinuria group. There were no differences in the serum hsCRP level among different GFR groups. The IL-6 levels of GFR 15~29 ml/min DN and GFR<15 ml/min DN groups were higher than those of the other groups, however, there were no differences among differ-ent groups. Conclusions:The patients with diabetic nephropathy micro-inflammatory state exists, and the inflammatory response has involved in the occurrence and progression of DN.
