目的：应用双腔带涤纶环导管进行血透的患者需要更换导管进行二次插管时探索原位再插管手术方法。方法在我科20例需要拔除导管再次插入导管的患者应用新的手术方法即把导管从涤纶环处剪断，将钢丝沿体内段管腔导入，退出体内段的导管，然后把新导管经钢丝插入，退出钢丝，涤纶环缝合于皮下固定。结果20例患者应用此种手术方法均成功地在原位进行导管更换，成功率100%。术后伤口无感染和首次插管相关并发症。结论本手术方法的优点是不需要另外再找血管，并且不需要使用扩管器；保护了患者有限的血管资源，减少了患者的痛苦。
목적：응용쌍강대조륜배도관진행혈투적환자수요경환도관진행이차삽관시탐색원위재삽관수술방법。방법재아과20례수요발제도관재차삽입도관적환자응용신적수술방법즉파도관종조륜배처전단，장강사연체내단관강도입，퇴출체내단적도관，연후파신도관경강사삽입，퇴출강사，조륜배봉합우피하고정。결과20례환자응용차충수술방법균성공지재원위진행도관경환，성공솔100%。술후상구무감염화수차삽관상관병발증。결론본수술방법적우점시불수요령외재조혈관，병차불수요사용확관기；보호료환자유한적혈관자원，감소료환자적통고。
Objective To explore in situ reincubation surgical method for hemodialysis patients need catheter replacement. Methods Application of this new method in 20 patients need reintubation. Cut the catheter at the polyester loop, put the wire through in vivo segment of catheter, fol owed by withdraw the catheter in vivo segment, indwel the new catheter and withdraw the wire, suture the polyester loop subcutaneously. Results We successful y replace catheter in situ in al the 20 patients without infections and complication associated with the initial incubation. The success rate is 100%. Conclusion This new method not necessarily need other vascular and usage of vascular expander, protect the limited recourses of vascular and reduce the suf ering of the patients.
